A Study of Sotatercept for the Treatment of Cpc-PH Due to HFpEF (MK-7962-007/A011-16)

This is a Phase 2, double-blind, randomized, placebo-controlled study to evaluate the efficacy and safety of sotatercept versus placebo in adults with Cpc-PH due to HFpEF. The objective of this study is to evaluate the efficacy, safety and tolerability of sotatercept versus placebo in adults with Cpc-PH due to HFpEF. Efficacy is measured by change from baseline in pulmonary vascular resistance (PVR, primary endpoint) and 6-minute walk distance (6MWD, key secondary endpoint).

A randomized, prospective study conducted on the basis of a social network (Instagram platform), two-stage. At the first stage, the online school "Save Your Heart" was announced and 945 applicants were enrolled in the training. The online school curriculum consisted of the following blocks: "Risk Factors for Cardiovascular Diseases (CVD)", "Healthy Nutrition for the Heart", "Cholesterol. What is important for the patient to know? "," Physical activity for the prevention of CVD "," Overweight and obesity "," Smoking as a risk factor for CVD "," AH: diagnosis "," AH: treatment "," Myocardial infarction: diagnosis and treatment". At the 2nd stage, 125 participants were selected by random randomization, then distributed into 4 training groups to provide training materials: group 1 (n = 31) - then text publications up to 4 thousand characters, group 2 (n = 31) - short video clips up to 5 minutes, group 3 (n = 33) - first text publications, then video clips, group 4 (n = 30) - video clips, then text publications. Before and after school, respondents from all four groups completed the HDKQ questionnaire.

Dietary Strategies for Remission of Type 2 Diabetes

In this project, the investigators will perform a multicenter randomised controlled trial to determine whether advice to consume a moderate, whole food-based low-carbohydrate high-fat (LCHF) ad libitum diet (CarbCount program) can produce and maintain equal remission rates of type 2 diabetes (T2D) as a nutritionally complete very-low-calorie formula diet followed by a energy-restrictive (i.e., calorie counting) diet (DiRECT principles). Within the principles of each approach, the dietary goals and change will be adjusted according to individual needs/capabilities conducive to long-term adherence. Furthermore, the investigators aim to determine whether the rate of diet-induced remission is reflected in/can be predicted by baseline or diet-induced changes in glucose variability (e.g., time-in-range measured by continuous glucose monitoring) and other factors such as anthropometric changes and genetic susceptibility. Each center will also conduct locally-lead standalone mechanistic research, including analyses of intra-abdominal/hepatic fat accumulation, adipose tissue biopsies and/or measurements of energy metabolism. Additionally, changes in medication use, nutritional status, cardiovascular disease risk, as well as adverse events, will be monitored.

Isometric Exercise for People With Raised Blood Pressure

High blood pressure affects many people in the United Kingdom. People with raised blood pressure (140-159/90-99 mmHg) are recommended to make changes in their lifestyle (e.g. smoking/alcohol/diet/exercise) and/or medication in order to reduce their blood pressure. Current knowledge suggests that a particular type of exercise - isometric exercise - can lower blood pressure. Isometric exercise involves holding a fixed body position for a short period of time. As most of the information about the benefits of this type of exercise comes from laboratory-based studies, researchers want to find out if it is possible for GP practices to offer NHS patients with clinically high blood pressure an isometric exercise plan to do at home and how it might affect their blood pressure over 6 months. They will also find out the experiences of those doing this type of exercise and whether it can be done consistently at home over time.
