Filter by:This study will compare single coil right ventricular defibrillation leads for implanted cardioverter/defibrillators (ICDs) in the apical and high septal locations. It is hypothesized that RV septal positions are at least as good as apical positions using formally measured defibrillation thresholds(DFTs), and that they may have long term advantages in reducing left ventricular dysfunction.

The goal of this study is to better characterize peripartum cardiomyoapthy or pregnancy-related cardiomyopathy by enrolling as many PPCM survivors as possible using both direct and web-based methods of recritment. Patients will anser a questionnaire regarding the onset, progression, treatment and follow-up of their diagnosis as well as the psychosocial aspects of PPCM.

Cardiac resynchronization therapy has been shown to be effective in reducing symptoms and mortality in heart failure patients. However, very few is known about the importance of the left ventricular pacing site. We study in a random order 11 different left ventricular pacing sites and compare their hemodynamics using pressure-volume catheters. The consequences on the surface ECG are also assessed.
The emphasis of this grant is to investigate ways to educate patients, not only to improve their knowledge about medications and diet that are important for better outcomes in heart failure, but also to try to understand how patients' beliefs about medication and diet affect their behaviour and to what extent the investigators can help patients change. Hypotheses: Heart failure patients who receive an enhanced educational intervention from their community pharmacist will: 1. have an absolute risk reduction in the number of events of 20%, 2. be more compliant, 3. demonstrate less health-related quality of life (HRQoL) impairment as measured by disease-specific and generic HRQoL instruments and 4. have lower costs/quality adjusted life years due to fewer total events per patient.
The metabolism of the heart provides the chemical energy needed to fuel ongoing normal heart contraction. Magnetic resonance spectroscopy is a technique used in a MRI scanner that can be used to measure and study heart metabolism directly but without blood sampling or obtaining tissue biopsies. One of the hypotheses this study aims to investigate is whether energy metabolism is reduced in heart failure and whether that contributes to the poor heart function.
This study is looking at overweight patients with chronic heart failure (CHF), to compare the effects of a modified fat diet with a reduced glycaemic load (diet 1); and a conventional low fat, high carbohydrate diet (diet 2) on: - insulin sensitivity (using the homeostasis model assessment [HOMA] model) - lipid profile - symptomatic status (6 minute walk distance and Heart Failure Quality of Life [HF QOL] Questionnaire) - body weight - inflammatory mediators (tumor necrosis factor [TNF] alpha, C-reactive protein [CRP], interleukin-6 [IL-6]) The hypotheses of this study are: - Diet 1 will be associated with lower insulin resistance than diet 2. - The lipid profile will be better in CHF patients on diet 1 than on diet 2. - Patients on diet 1 will have a better symptomatic status than patients on diet 2. - Diet 1 will maintain body weight in patients with CHF as well as diet 2. - Diet 1 will suppress the expression of TNF-alpha, CRP and IL-6 more than diet 2.
