Self-assured Parents - a Parenting Support Program for Immigrant Parents With Teenage Children Living in Deprived Areas

Parents have the primary responsibility for child socialization and development, but not all parents have the same possibilities to promote their children's positive development. Immigrant parents living in deprived areas often worry about their children's safety and future, at the same time as they have difficulties facilitating the best development potential for their children. Social services can help parents and their children to attain more promising developmental outcomes through focus on early preventive parenting support efforts, but these efforts need to be culturally tailored for the best possible results. For this reason, social services in the municipality of Örebro developed a culturally sensitive parenting support program aimed at immigrant parents living in deprived areas, who are worried that their children (age 12-18) engage in or will be exposed to harmful environments. The Self-Assured Parenting Program (SAP) offers support to these parents by building on protective factors and strengthening parents in their parenting through focus on parenting competence and parent-child communication. The purpose of SAP is to increase parents' self-confidence and communication between parents and their teenagers as well as to reduce parents' worries through activities that have a clear focus on empowerment and knowledge of child development. This multi-design project aims to test the implementation and effect of TF in Örebro and other Swedish municipalities with similar problems through observation, interviews with parents and groupleaders/managers as well as longitudinal effect measurements of parenting competence, parent-child communication and worries about their children's psychosocial development. This project will allow a partnership between social workers and researchers to be formed in order to generate practice-based evidence about implementation of support to deprived parents, which can be used in the context of everyday social service practice.

Added Value of Preoperative Contrast Enhanced Mammography in Staging of Malignant Breast Lesions

Patients diagnosed with breast cancer after routine assessment with digital mammography and ultrasound and scheduled for primary surgery are included in the trial. The patients are randomized 1:1 to go through additional contrast enhanced mammography (CEM) or no further imaging preoperatively. Primary endpoint is rate of patients with change of treatment: a) mastectomy instead of partial mastectomy due larger unifocal or multifocal extent, b) partial mastectomy instead of mastectomy due to improved demarcation of the tumour area, c) bilateral surgery instead of unilateral due to findings of contralateral cancer, d) neoadjuvant therapy instead of primary surgery due to more advanced disease. Rate of reoperation and rate of avoidable mastectomies are secondary endpoints.

Purpose: The emergence of a new coronavirus SARS-CoV-2 causing a novel infection in the human race resulting in a world-spanning pandemic came as a surprise and at a tremendous cost both for individual human lives as well as for the society and the health care sector. The knowledge on how this new infection affects both the mother and the unborn child as well as the outcomes for the mother and the child in the long run are unknown. What is known is based on case-reports and small case-series solely. Both the coronaviruses causing Middle East Respiratory Syndrome (MERS) and Severe Acute Respiratory Syndrome (SARS) can cause a threat to pregnant women and their offspring, which leads to the question whether this could be the case also for SARS-CoV-2. Aims: To establish a biobank of biological material from infected as well as non-infected pregnant women and their offspring. To combine this biobank with Swedish quality and health care registers, computerized patient charts and questionnaire data, enabling both short-term follow up, such as obstetric outcomes, as well as long-term outcomes both for mother and child. To study how the pandemic situation affects both the mother and her partner in their experience of pregnancy, childbirth, and early parenthood. Design: A national Swedish multicentre study. Women are included when they have a positive test for SARS-CoV-2 or a clinical suspicion of coronavirus disease 2019 (COVID-19) (COVID-19 group). Pregnant women without COVID-19 symptoms will be included at their routine visits (Screening group). Blood samples and other biological material will be collected at different time-points. Additional predictors and outcomes are collected from the Swedish Pregnancy Register as well as obligatory Swedish health registers. The biobank and its linkage to health registers through the Swedish personal identification number will enable future research. Child development will be followed during the first year of life by questionnaires to the parents. Womens' and their partners' experience of childbirth and parenthood will be studied in form of questionnaires as well as in form of interviews. Conclusion: This project will help obstetricians and neonatologists better recognize clinical manifestations of the virus, identify possible risk factors during pregnancy and tailor therapies alongside providing right level of surveillance and management during pregnancy, delivery, and child health care.

Supplementation After Bariatric Surgery

The aim of this study is to assess medication adherence to vitamin and mineral supplements after bariatric surgery and identify factors that may influence adherence. All study participants are prescribed life-long treatment with daily oral vitamin B12 and Calcium/Vitamin-D. Menstruating women are also prescribed daily oral iron supplementation. Post-operative adherence rate is retrieved from Pharmacy refill data, collected from the Swedish Prescribed Drug Register, the five first years after bariatric surgery.
