Introduction: Early and rapid diagnosis of etiology is often an important part of saving the lives of patients in emergency department. Chest CT is an important examination method for emergency diagnosis because of its fast examination speed and accurate localization. Traditional medical imaging diagnosis relies on radiologists to report in a qualitative and subjective manner. Through the interdisciplinary combination of clinical, imaging and artificial intelligence, the integration of multi-omics data, the construction of large-scale language models, and the construction of the auxiliary diagnosis support system of "one check for multiple diseases" provide new ideas and means for the rapid and accurate screening of emergency critical diseases. Method: Study design Investigators retrospectively collected cardiovascular, respiratory, digestive, and neurological CT images, demographic data, medical history and laboratory date of emergency department patients during the period from 1 January 2018 and 30 December 2024. Regularly carry out standardized follow-up work, and complete the collection and database establishment of clinical-imaging multi-omics data of patients attending emergency department.The inclusion criteria are:1. adult emergency patients with cardiovascular, respiratory, digestive, and nervous system diseases; 2. These patients had CT images. Patients with incomplete clinical or radiographic data were excluded from the analysis. Regularly carry out standardized follow-up work, and complete the collection and database establishment of clinical-imaging multi-omics data of patients attending emergency department. Based on the collected medical text data, an artificial intelligence large-scale language model algorithm framework is built. After the structure annotation of chest CT images is performed by doctors above the intermediate level of imaging, the Transformer deep neural network is trained for CT image segmentation, and a series of tasks such as structural structure segmentation, damage detection, disease classification and automatic report generation are developed based on Vision Transformer self-attention architecture mechanism. A multi-disease diagnosis and treatment decision-making system based on chest CT images, clinical text and examination multimodal data was constructed and validated. Disscusion Emergency medicine deals mainly with unpredictable critical and sudden illnesses. Patients who come to the emergency department for medical treatment often have acute onset, hidden condition, rapid progress, many complications, high mortality and disability rate. Assisted diagnosis systems developed by combining clinical text, images and artificial intelligence can greatly improve the ability of emergency department doctors to accurately diagnose diseases. This study fills the blank of CT artificial intelligence aided diagnosis system for emergency patients, and provides a rapid diagnosis scheme for multi-system and multi-disease. Finally, the results will be transformed into clinical application software and used and promoted in clinical work to improve the diagnosis and treatment level.
The goal of this randomized control trial is to test the efficacy and safety of a bioglass wound dressing in patients with chronic wound in different stages. The main question it aims to answer are: - Whether the bioglass wound dressing have better efficacy and safety than blank control in treatment of chronic wound in necrosis stable stage? - Whether the bioglass wound dressing have better efficacy and safety than blank control in treatment of chronic wound in granulation growth stage? Participants will be assigned with a 1:1 ratio into intervention group and control group. Participants of intervention group will receive bioglass wound dressing in addition with clinical optimal treatment plan. Participants of control group will only receive clinical optimal treatment plan.

Finerenone is a new selective nonsteroidal mineral corticoid receptor antagonist (MRA), nowadays it's widely used in type 2 diabetes (T2DM) patients with chronic kidney disease (CKD), the newest trial shows finerenone improve the cardiovascular outcomes among patients with T2DM and CKD especially reduce the risk of hospitalization for heart failure. In patients with diabetic nephropathy, finerenone resulted in lower risks of CKD progression and cardiovascular events. Finerenone shows great potential therapeutic effect in chronic heart failure (CHF) patients with or without T2DM and CKD compared to eplerenone, but there is still no real world study on finerenone in patients with heart failure with reduced ejection fraction (HFrEF) and it's unclear about the effect of finerenone in CHF patients without T2DM and CKD. The investigators will conduct a study to demonstrate the efficacy and safety of finerenone in HFrEF patients compared to other MRAs.
This study has been designed as a 52-week, randomized double blind placebo controlled multicenter clinical trial. The study aims to compare the efficacy and safety of two treatment strategies in IgG4-RD patients with re-elevation of serum IgG4 level during maintenance remission period: basic maintenance treatment group (continue use of basic maintenance treatment of IgG4-RD) and enhanced treatment group (use low dose mycophenolate mofetil as an add-on therapy of basic maintenance treatment of IgG4-RD).
The ribs are rigid and flexible structures that make up the chest skeleton and are a set of twelve paired bones. After Blunt trauma rib fractures incidence is about 30 to 40% [1]. Due to trauma, (4th -9th) ribs are often affected and the fracture point of the ribs is often on the midaxillary line. In ( 1st&2nd) ribs fractures subclavian vessels and brachial plexus damage may occur. In fractures in the lower ribs (8th-12th ribs), intra-abdominal organs injury such as liver, spleen and kidney should be investigated. 11-12. ribs fractures are rare because the ribs are more flexible (floating). Pain is the most important symptom indicating rib fracture, and it usually increases with coughing, breathing, and movement. Broken rib ends can be felt with palpation with sever tenderness at the site of fracture. Pneumothorax and/or hemothorax may develop as a result of the sharp end of the broken rib causing lung parenchymal damage[2].
Serious mental illnesses are all chronic illnesses such as schizophrenia, schizoaffective disorder, and other psychotic disorders, major depression, and bipolar disorder, which cause impaired functioning in areas such as activities of daily living, self-care, social relationships, professional and academic life, quality time, etc. These chronic illnesses are a significant source of stress for the patient and family members. With the transition to community-based practices in the care of chronic mental illnesses, t he duties of families have increased and as a result, have had significant negative effects on caregiver and family functioning. Caregivers face many challenges and these challenges cause caregivers to experience feelings such as anxiety, stress, fear, guilt, helplessness, hopelessness, powerlessness, loss, and anger. It is understood that families are in significant need of psychosocial support interventions that will enable them to develop effective coping methods and reduce stress and anxiety. Themotivationall Interviewing Technique is one of the methods that can be applied to the caregivers of individuals diagnosed with chronic mental illness to adapt to the situation and cope with the difficulties they experience. Motivational Interviewing does not deal with issues such as teaching new information or ensuring acceptance of the past. It focuses on the individual's current stress, anxiety and dilemmas, goals, and plans. Studies show that any intervention that will contribute to the caregiver's coping attitude and psychological distress level will also support the empowerment of the sick individual. For this purpose, the study was planned to determine the effect of motivational interviewing intervention for caregivers of individuals with serious mental disorders on psychological distress and coping attitudes.

Despite conflicting and contradictory evidence regarding its efficacy, some wound care centres have advocated and adopted ozone for treating DFU. However, there are gaps in the application of topical ozone therapy. Reported no significant impact on the healing process of DFU, and not all said that topical ozone can enhance the healing process. This study aimed to compare the efficacy of topical ozone therapy in conjunction with standard wound care versus routine wound care alone in treating DFU. The wound, ischemia, and foot infection (WIFI) scale was used to measure wound size, the tissue survival rate at DFU, infection, peripheral microcirculation, glycemic control, Hba1c control, and wound healing.
