Filter by:Aortic aneurysms are the major disease processes affecting the aorta and becoming a relatively common cause of death because of rupture or dissection. The most common location for aneurysms is the infrarenal abdominal aorta, followed by the ascending thoracic aorta. Unlike coronary heart disease, the incidence of abdominal aortic aneurysm (AAA) in the United States and Europe has been increasing, and this increase may not be due to higher levels of screening for this condition alone. Aortic diameter is central to the diagnosis of aortic aneurysm. Furthermore, it was demonstrated that non-AAA patients with an enlarged diameter of the infrarenal aortic diameter are also at high risk for all-cause mortality. And aortic root dimension was associated with several coronary artery disease (CHD) risk factors and measures of subclinical disease and was predictive of incident congestive heart failure (CHF), stroke, cardiovascular disease (CVD) mortality, and all-cause mortality, but not of incident MI. Up to now, there are limited studies on the epidemiology of aortic diameter, especially in Chinese population.
Purpose of the study: To compare three different regimes for children newly diagnosed with type 1 diabetes: (1) traditional hospital based care, (2) intensive psychological support to the family, and (3) home based family psychosocial support. A further aim is to identify families were the child runs the risk of decreased metabolic control and to give those families increased support. Specific objectives: The main objectives of the study are: 1. to analyse the effects on child health of family-based home care compared to traditional hospital-based care, and 2. to analyse the effects on child health of family-based psychological treatment compared to traditional hospital-based treatment. The main focus will be on child health and changes in child health. Several variables will be collected for these purpose (see instruments, below), the main variable being the child's metabolic control. We will also analyse effects on the amount and content of (1) parental efforts (absenteeism from work; time use which is conditional on the child's diabetes etc), and (2) formal health care utilisation of the home-based care and psychological treatment, respectively, compared to traditional care.

Cholesterol-lowering drugs called statins improve the functioning of the endothelium, and help prevent heart disease. We are testing whether statins improve endothelial function more in the arteries that have worse endothelium to begin with. One of the functions of the endothelium is to help control how blood vessels dilate (expand) or contract (narrow) in different situations. This affects how blood flows through those vessels. Magnetic resonance imaging (MRI) can be used to evaluate endothelial function in the arms and legs noninvasively. You are being asked to participate in a study using MRI to test whether a 3-week course of statins improves the endothelium of legs more than it improves the endothelium of arms. Adults with type 2 diabetes may join.

This study describes a controlled randomized clinical trial to evaluate the effect of inspiratory muscle training in patients with diabetic autonomic neuropathy.
We are comparing the blood pressure-lowering effects of two marketed medications, Coreg CR and Toprol XL. Although both drugs reduce blood pressure by blocking the action of noradrenaline on beta-receptors in the blood vessels, Coreg CR also blocks alpha-receptors, which may provide added blood pressure-lowering. In addition, Coreg CR may have anti-oxidant actions. Cells which line blood vessels (termed "endothelial cells") make nitric oxide (NO), which relaxes the muscle cells encircling the blood vessels, causing a reduction in blood pressure. When body cells use oxygen, they normally produce "free radicals", which can destroy NO,leading to high blood pressure, heart damage and worsenimg of diabetes. Antioxidants remove free radicals and prevent or repair this damage. In this study we will measure endothelial cell function, blood vessel wall stiffness, NO in exhaled breath, and blood levels of substances which reflect NO production and destruction to determine if a pure beta-blocker (Toprol XL) differs from an alpha/beta blocker (Coreg CR) in these effects. We will also examine the mechanism by which such differences might occur.
This study has been designed to test whether using Thymoglobulin with low dose Cyclosporine and early steroid dosage reduction will minimize both kidney rejection and the development of new onset diabetes mellitus after renal transplant.
Recently a new category of antihyperglycemic therapy aiming to modulate the incretin system has emerged. These drugs induce insulin secretion without inducing hypoglycemia. The effect of the incretin modulators drugs on hypertension, arterial stiffness, inflammation and oxidative stress parameters have not been fully investigated yet.GLP-1 analogue has been suggested to have an effect on endothelium and the development of hypertension. Nystrom et al have demonstrated that GLP-1 improves endothelial dysfunction in a small group of type 2 diabetes subjects, with coronary heart disease. We hypothesize that DPP-4 inhibitor will have an effect on hypertension and arterial stiffness by effect on the NO pathway.The aim of this study is to investigate the effect of two insulin inducers drugs, sulfonyl urea and DPP-4 inhibitor on 24 hours blood pressure monitoring, arterial stiffness, oxidative stress and inflammation.
It has been observed that several of patients having prolonged or complicated course of acute viral hepatitis have underlying diabetes. It is possible that with impaired hepatocyte regenerating capacity, these patients run a more prolonged and complicated course. We hypothesize that acute hepatitis infection has a prolonged and complicated course among diabetic patients.
