Effects Of Losartan On Myocardial Structure In Diabetic Hypertensive Patients With Left Ventricular

The anthypertensive treatment with Losartan may have benefits beyond blood pressure reduction on myocardial structure and function in hypertensive diabetic patients. We will evaluate the effect of losartan treatment on structural characteristics of myocardium in hypertensive diabetic patients: 1. left ventricular mass, intraventricular septal thickness, fractional shortening. 2. myocardial qualitative alteration and heterogeneity of myocardial tissue that describes myocardial texture and echodensity, related to collagen deposition; myocardial qualitative alteration will be evaluate by ultrasonic myocardial integrated backscatter signals (IBS) both as peak end diastolic signal intensity and as cardiac cyclic variation 3. alteration of diastolic function as studied by Doppler flow velocities across the mitral valve (Pulse Wave Doppler) and pulse wave Tissue Doppler Imaging parameters 4. aortic strain and distensibility (that is in relation with LVH) 5. epicardial adipose tissue measurement (this parameter is related to the visceral fat and may be an easy method to indicate patients with high cardiovascular risk).

A Barrier Assessment Tool (BAT) will be validated to correlate with provider and practice adherence to diabetes care guidelines. The network is part of the Crozer-Keystone Health System located in Delaware County, Pennsylvania, in the southeast quadrant of the Philadelphia metropolitan region.

Novel Home Care Device for High-Risk Diabetic Patients

More than half of all lower extremity amputations are in persons with diabetes. These patients suffer from severe, diabetes-induced, peripheral, sensory neuropathy and, thus they frequently do not protect their feet from repetitive shear stress or traumatic episodes and ulceration often ensues. We have previously shown that the temperature profile of the plantar aspects of the foot provides a reliable warning of tissue injury and can be effectively used as a preventive modality. In this study we propose to further develop and clinically test a novel infrared-based temperature instrument (TempTouchRM®) that is intended for home use by high-risk diabetic patients. This step-on remote monitoring device will serve as an early warning system for impending ulcers and Charcot fractures. The study’s central hypothesis is that the TempTouchRM device will reduce the incidence of ulcers by providing an accurate, simple, and effective approach to monitor changes in foot temperatures.

Many adipokines are thought that related with metabolic disturbance such as glucose intolerance, dyslipidemia, and insulin resistance. It is reported that regular physical training could prevent the progression of diabetes from prediabetes and improve the insulin resistance. After physical training, many adipokine levels were changed due to improving insulin resistance. In this study, we will examine the various adipokine levels such as adiponectin, RBP-4, and adrenomedullin after exercise training through exercise prescription in IFG and IGT patients.

Diabetes Quality Improvement Program on Diabetes Case Management Program 2001

In order to assess more completely the levels of diabetes care delivered in Taiwan, we now have nationally standardized performance measures that can evaluate quality of diabetes care accurately and reliably. These performance measures were seriously reviewed and discussed in the aspects of their practicability, feasibility and compliancy, through a series of nation-wide consensus meetings by a number of multidisciplinary professionals of Taiwan Diabetes Care Teams ( TDCTs ), before delivering into the clinical settings ( 10 ). Conceptually, these measures will enhance uptake of research into practice and may ultimately improve diabetes care and clinical outcomes. Along with the serial developments mentioned above, the BNHI ( the sole healthcare insurance organization in Taiwan ) has been fully involved, from the very beginning, in the formation and development of these performance measures which have totally been included in the development of the Diabetes Case Management Program ( DCMP ) 2001 later on. The DCMP 2001 is a unique program that has been delivered into the nation-wide clinical practices since Dec. 2001 in Taiwan. It has linked the quality of diabetes care to the monthly income ( reimbursed from BNHI ) of the registered clinical diabetes educators ( i.e., physicians, nurses and dietitians ) for enhancing and assuring the complete implementation of these performance measures in diabetes care. It has been recognized that a consensus on measures at national level could provide a method for assessing care within and across healthcare settings while providing a meaningful mechanism for quality improvement. In this proposal, we are going to develop the Diabetes Quality Improvement Program ( DQIP ) based on the DCMP 2001 in Taiwan. Initially, we are going to establish the continuous diabetes quality monitoring system and then to evaluate the (1) the lifestyles and eating habits and dietary compositions in type 2 diabetes mellitus (2)The relationships between lifestyles and eating habits and dietary compositions in diabesity.

We hypothesize that autologous bone marrow stem cell transplantation will promote β-cells regeneration by directly differentiated from the transplanted BMMCs or stimulated local stem cells regeneration and thus decrease or eliminate the need of exogenous insulin and improve β-cells function.
