Patients with COPD who deteriorate to Respiratory Failure pose a challenge to the intensivist both in ventilating an obstructed patient and in weaning from mechanical ventilation. Currently there are no accepted extubation criteria in COPD patients that could predict successful weaning from mechanical ventilation. In this study we will attempt to correlate spirometric data to eventual weaning from mechanical ventilation.

"Normal Coronary Artery" With Slow Flow Improved by Adenosine Injection, Dipyridamole Treatment and Clinical Follow-up

The investigators will investigate patients undergoing coronary angiography with normal coronary but slow blood flow that was normalized after adenosine injection into the coronary artery. The investigators believe that they have small vessel coronary disease which is the initial presentation of coronary disease prior to anatomical narrowing. To alleviate this phenomenon the investigators intend to examine the long-term clinical response of these patients to treatment with dipyridamole, a well-known medication with almost no side-effects that increases the level of adenosine in the tissue.

Subjects will arrive at the investigators' gait laboratory for assessment. A sub-group of the subjects, will receive a monitoring device, to be worn at home for three days.

Effectiveness of Intracoronary Injection of Eptifibatide in Primary Coronary Intervention in STEMI Patients

The achievement of high local concentration of Eptifibatide, a GP 2b3a inhibitor,via direct intracoronary injection, promotes (in vitro) clot disaggregation. It remains unclear if it is of superior benefit than the routine intravenous administration of these agents. In patients presenting with acute myocardial infarction, and undergoing primary coronary intervention, intracoronary administration of Eptifibatide may increase local drug concentration by several orders of magnitude and promote clot disaggregation with a minimal increase in systemic drug concentration, and in that way enhancing myocardial perfusion and survival.
