Alternatives to Dental Opioid Prescribing After Tooth Extraction

The goal of this clinical trial is to test a three-item intervention in oral surgeons who remove teeth. The main questions it aims to answer are: - Can the intervention reduce opioid prescriptions to adolescents and young adults after tooth removal? - Do oral surgeons' beliefs about the intervention and opioid prescribing change? - Do patients that report using opioids after tooth removal have different experiences than patients that do not? Oral surgeon participants will: - Attend a 1-hour education session with a trained pharmacist - Receive patient instructions and blister packs of pain medicine to give to patients - Complete 2 surveys about feasibility and appropriateness Patient participants will complete a survey about pain and medication use after having a tooth removed. Researchers will compare the intervention to usual care to see if it reduces opioid prescribing.

Immediate Implant Placement Using Volume-Stable Collagen Matrix Combined With rhPDGF-BB

The goal of this study is to test a volume-stable collagen matrix in combination with recombinant human platelet-derived growth factor-BB, specifically in patients who have an immediate implant placement for teeth #5-#12. The main question this study aims to answer is does the recombinant human platelet-derived growth factor-BB accelerate the formation of blood vessels and augment tissue volume around the immediate implant. Participants will be asked to undergo immediate implant placement using volume-stable collagen matrix in combination with recombinant human platelet-derived growth factor-BB, as well as attend 11 study visits over the course of 1.25 years.

Ridge Preservation Using Different Techniques

Soft tissue contour and radiographic evaluation of alveolar ridge preservation using different techniques Background: Following tooth extraction, the alveolar ridge undergoes an inevitable remodeling process which influences future implant therapy or prosthetic rehabilitation in the edentulous area. In an attempt to attenuate the loss of hard and soft tissue after tooth loss, alveolar ridge preservation (ARP) immediately after complete tooth extraction could minimize the need for ancillary ridge augmentation or soft tissue grafting. Different techniques and barrier membranes has been proposed to achieve the sealing of extraction socket. Aim: To investigate the effect of different techniques and barrier membranes for the soft tissue contour and morphological change of alveolar ridge after ARP Methods: The study was designed as a randomized controlled trial and recruited patients, who require ARP for the purpose of implant placement or prosthodontic rehabilitation. After the tooth extraction, patients were randomly allocated to one of the following groups: ridge preservation with a xenogeneic bone substitute and (a) spontaneous healing (control), (b) covered with a free palatal graft, (c) or covered with pedical palatal graft, (d) covered with a collagen membrane, (e) covered with a non-resorbable high-density polytetrafluoroethylene membrane. 2 weeks, 4 weeks, 12 weeks, and 24 weeks following tooth extraction and ARP, clinical profilometric and radiographic evaluations were performed to analyze the change of hard and soft tissue contour. Moreover, and the need for additional guided bone regeneration (GBR) or soft tissue augmentation were assessed prior implants or fixed prosthesis placement.

On Adequacy of Referrals to Oral and Maxillofacial Surgery

Currently, an ongoing generational shift of the dental community in Sweden originates from an increased number of retiring dentists. Meanwhile, a declining number of dental professionals puts a strain on the care system and reduces the availability of dental care to patients in need of treatment. With fewer experienced dentists, available to guide newly graduated dentists, an increased number of referrals to specialist care units might be a consequence. E.g. referrals to oral and maxillofacial departments originating from a need of assistance in more advanced cases of assessments, treatment planning and therapies. The decreasing number of dentists is also possible to negatively affect the recruitment to specialized dental units, which in turn further decreases treatment availability. When the need for dental care exceeds the capacity of the dental care providers, a possible solution might be to minimize the number of referrals possible to treat in general dentistry, thus prioritize patients truly in need of specialist care. Hence, a reduced waiting time and improved accessibility to an adequate treatment. The aim of this study is to explore the suitability of referrals concerning exodontia to the departments of oral and maxillofacial surgery in southern Sweden. The primary objective is to assess the level of difficulty in referrals on dental extractions. Secondary objectives are assessments of outcome from treatment and comparison of the suggested treatment of the referral with the outcome. The hypothesis is that the level of difficulty of the required treatments in referrals to oral and maxillofacial departments in southern Sweden is low and constitutes a contributing factor to prolonged waiting queues in specialized dentistry.

Evaluation of β-tricalcium Phosphate/Calcium Sulfate Versus Allografts

Thirty healthy patients will be selected from the outpatient clinic of the Oral and Maxillofacial Surgery Department, Faculty of Dentistry, Mansoura University, to be included in this study with non-restorable posterior teeth.Patients will be selected according to the following criteria: 1) having a non-restorable posterior tooth. 2) being healthy, with no systemic diseases that might contraindicate receiving a dental implant. 3) having good oral hygiene. 4) aged between 18 to 45 years old. 5) having sufficient alveolar bone volume and height at the future implant site.Patients were classified into three groups: - Group I: ten extraction sockets left for normal healing (blood clot). - Group II: ten extraction sockets filled with Alloplast bone grafting material (EthOss, Ethoss Regeneration Ltd, Silsden, UK). - Group III: ten extraction sockets filled with Allograft bone grafting material.Evaluation and Follow-up All patients will be evaluated clinically and radiographically during a follow up period of 4 months, where the following criteria will be evaluated: A- Clinical evaluation: - Patients will be evaluated at 1 week, 4 weeks and 8 weeks. - Clinical evaluations of soft tissue healing, and keratinized tissue replacement will be assessed on the basis of the healing index proposed by Landry et al12. - The scores will assess healing on the basis of redness, presence of granulation tissue, bleeding, suppuration, and epithelization. - The surgical region will be examined clinically, if there is pain, redness, tissue resorption and bone graft stability. B- Radiographic evaluation: - Cone Beam Computed Tomography scan will be taken immediately after implant placement, and after 12 months. - All patient scans will be taken by a Planmeca ProMax 3D unit (Planmeca OY, Helsinki, Finland) using fixed imaging parameter at every scan. All DICOM data will then be analyzed using On Demand 3D software (Version 1, Cyber Med, Seoul, South Korea). - The vertical and horizontal measurements will be measured. - The apico-coronal dimension will be calculated, the bottom of the socket will be considered the most apical point, whereas the alveolar bone margin will be considered the most coronal point of the socket bone wall. C- Histological evaluation: - Bone samples will be collected with trephine bur before implant placement, then fixed in 10% neutral buffered formalin for 24 hours, then transferred to 14% neutral buffered Ethylenediaminetetraacetic acid (EDTA) for decalcification 13. After confirming adequate decalcification, the specimens will be processed and embedded in paraffin blocks in the Pathology department, faculty of Medicine, Mansoura university for histological assessment, then stained with haematoxylin and eosin (H&E) as a routine stain to evaluate the quality of newly formed bone.

Treatment of Alveolar Osteitis: Traditional or Regenerative?

Alveolar osteitis, commonly referred to as dry socket, is a common painful postoperative complication following tooth extraction. It is defined as pain in and around an extraction socket which increases at any time between one and three days following extraction, accompanied by partial or total loss of the blood clot from the socket, with or without halitosis (Blum, 2002). The incidence of the complication varies from 1-5% for routine extractions up to approximately 30% for third molar extractions. There are a variety of factors that contribute to the incidence of dry socket including tissue trauma, smoking in the early postoperative period and the oral contraceptive pill. Although it is a self- limiting condition, symptoms can be very debilitating for patients and therefore following diagnosis, immediate treatment should be provided to expedite resolution and to their improve quality of life during the healing period. Treatment options for dry socket vary from rinsing of the socket, regular analgesia and sometimes placement of an obtundent, sedative dressing such as Alveogyl or a zinc oxide eugenol alternative. This study will aim to compare two treatments for patients who present with alveolar osteitis (dry socket) to Dublin Dental University Hospital after tooth extraction. Following randomisation, one of two treatment modalities will be used, either a experimental treatment (Bio-PRF) or a positive control (Alveogyl). The results of this study will enable us to learn more about the efficacy of these treatment options for dry socket and how they affect patients.

Ridge Preservation Following Tooth Extraction

An animal study and our pilot human study demonstrated that placing a smooth-surface titanium bone screw (TBS) over the buccal plate of an extraction socket resulted in less dimensional changes of the alveolar ridge along with no adverse effects. The purpose of this randomized clinical trial is to investigate the effectiveness of TBS application for ridge preservation in comparison to (1) unassisted/non-interventional socket healing and (2) alveolar ridge preservation (ARP) approach using allografts and a membrane.
