NCT ID: NCT02737761 Completed - Clinical trials for Congestive Heart Failure

Researching Emotions And Cardiac Health

REACH
Start date: January 2015
Phase: N/A
Study type: Interventional

The purpose of the study is to understand how positive emotions (e.g., optimism, happiness) are associated with health behavior adherence in patients with heart failure (HF), as well as whether performing exercises to improve positive emotions may help to improve health behavior adherence as well.

NCT ID: NCT02702752 Active, not recruiting - Atrial Fibrillation Clinical Trials

Dynamic Changes in SDF-1α Levels in Acute and Stabilized Heart Disease

DYNASDY
Start date: November 2015
Phase: N/A
Study type: Interventional

The aims of the present study are to measure dynamic changes over time in the plasma level of SDF-1α in patients with confirmed diagnoses of AMI, AF and CHF and in so doing: 1. Monitor the plasma level of SDF-1α at the diagnosis, during and after the long term treatment of AMI, AF and CHF. The SDF-1α level of patients is expected to decrease during treatment of the aforementioned conditions. 2. Correlate the plasma level of SDF-1α in relation to clinical, biochemical characteristics of Acute Myocardial Infarction(AMI), Atrial Fibrillation(AF) and Congestive Heart Failure (CHF). In so doing, the investigators expect that SDF-1α will correlate with the severity of heart disease. 3. Study the dynamic of SDF-1α pertaining to its property as prognostic indicator for the long term follow up risk of readmission and mortality of patients diagnosed with AMI, AF or CHF.

NCT ID: NCT02700425 Completed - Clinical trials for Congestive Heart Failure

His Bundle Pacing Versus Coronary Sinus Pacing for Cardiac Resynchronization Therapy

His-SYNC
Start date: May 17, 2016
Phase: N/A
Study type: Interventional

The goal of this study is to compare the effectiveness of pacing from a physiologic His bundle (HB) lead position versus with the standard coronary sinus (CS) lead position in subjects with heart failure undergoing cardiac resynchronization therapy (CRT). While placement of left ventricular leads via the coronary sinus has anatomic limitations, we hypothesis that the achievement of QRS narrowing with His bundle capture will be superior for improving systolic function by echocardiographic indices (ejection fraction and strain) and quality of life and decreased rehospitalization and mortality.

NCT ID: NCT02700191 Withdrawn - Clinical trials for Congestive Heart Failure

Cardiovascular Diagnosis Using µ-Cor - an Interventional Pilot Trial

Start date: May 4, 2016
Phase: N/A
Study type: Interventional

Subjects that are being treated at the Congestive Heart Failure clinic in Nazareth will be given the µ-Cor system to be added to their standard care, and randomized 1:1 to an interventional arm or a control arm.

NCT ID: NCT02686749 Terminated - Atrial Fibrillation Clinical Trials

Catheter Ablation vs. Medical Therapy in Congested Hearts With AF

CATCH-AF
Start date: June 2016
Phase: Phase 4
Study type: Interventional

This study is a multi-center, randomized, unblinded, clinical trial. The objective is to determine if catheter-based atrial fibrillation (AF) ablation is superior to medical treatment in patients with impaired left ventricular (LV) function who have been diagnosed with symptomatic AF within the past 12 months.

NCT ID: NCT02682251 Completed - Clinical trials for Congestive Heart Failure

Engaging Patients in Heart Failure Management

Start date: February 2016
Phase: N/A
Study type: Interventional

An interventional study to empower congestive heart failure patients who have a cardiac resynchronization therapy-cardiac implantable electronic device (CRT-CIED) by using a personal health record (PHR) to directly message them their device data and send alerts based on percent left ventricular pacing.

NCT ID: NCT02673892 Terminated - Clinical trials for Chronic Obstructive Pulmonary Disease

Patient Oriented Discharge Summary Impact Study

PODS
Start date: February 2016
Phase: N/A
Study type: Interventional

Improving the patient experience has become a major focus of quality improvement efforts in Ontario and in health systems worldwide. However, our existing knowledge base is relatively under-developed, particularly in how patients experience care as they transition from one care setting to another and the relationship between patient experience and clinical outcomes. The Patient Oriented Discharge Summary (PODS) is a discharge instruction tool created by patients, caregivers, health-care providers and design experts. It provides a written template for providers to engage patients and caregivers when reviewing important instructions on medications, activity and diet restrictions, follow-up appointments and worrisome symptoms warranting emergency care following admission to hospital. The PODS also uses plain and simple wording, large fonts, pictograms, and includes white space for patients to take notes and provides the option for translation of major headings into the most common spoken languages. The PODS impact study will study the impact of using the PODS versus usual discharge instructions on patient experience and health outcomes in a provincial-wide randomized study across acute care and rehabilitation hospitals.

NCT ID: NCT02660385 Completed - Pain Clinical Trials

Insomnia Self-Management in Heart Failure

HSS
Start date: March 7, 2016
Phase: N/A
Study type: Interventional

Chronic insomnia may contribute to the development and exacerbation of heart failure (HF), incident mortality and contributes to common and disabling symptoms (fatigue, dyspnea, anxiety, depression, excessive daytime sleepiness, and pain) and decrements in objective and subjective functional performance. The purposes of the study are to evaluate the sustained effects of CBT-I on insomnia severity, sleep characteristics, daytime symptoms, and functional performance over twelve months among patients who have stable chronic HF and chronic insomnia. The effects of the treatment on outcomes of HF (hospitalization, death) and costs of the treatment will also be examined. A total of 200 participants will be randomized to 4 bi-weekly group sessions of cognitive behavioral therapy for CBT-I (behavioral was to improve insomnia and sleep) or HF self-management education. Participants will complete wrist actigraph (wrist-watch like accelerometer) measures of sleep, diaries, reaction time, and 6 minute walk test distance. They will also complete self-report measures of insomnia, sleep, symptoms, and functional performance. In addition the effects on symptoms and function over a period of one year.

NCT ID: NCT02646540 Completed - Clinical trials for Congestive Heart Failure

AiDing Diuresis wIth Tolvaptan (ADD-IT)

ADD-IT
Start date: January 2016
Phase: Phase 1
Study type: Interventional

The purpose of this study is to test the use of tolvaptan (commercial name Samsca) when given in addition to intravenous (IV) diuretics early during an episode of acute heart failure. It will be compared to a high dose of IV diuretics alone or the use of metolazone in addition to IV diuretics. The investigators are looking to see if there is improvement in the symptoms and outcomes of persons with acute heart failure.

NCT ID: NCT02632656 Active, not recruiting - Clinical trials for Congestive Heart Failure

Plasma ExtrAcellular RNAs and Biomarkers of Heart FaiLure During Decongestion: PEARL-HF Study

PEARL-HF
Start date: December 2015
Phase:
Study type: Observational

The primary objective is to measure the association between extracellular RNA (ex-RNA) levels in plasma in patients receiving aggressive outpatient therapy for CHF with (1) cardiac remodeling and (2) cardiovascular events. The investigators will follow patients during standard medical therapy for CHF to assess changes in ex-RNA levels in the plasma, and how these are associated with cardiac remodeling (by cardiac imaging) and outcomes.
