Clinical Trial Summary

The Richmond Health and Wellness Program (RHWP) is an innovative and emerging nurse-led inter-professional wellness and care coordination model that represents a holistic and integrated approach to community based care. The model addresses unmet health care needs of vulnerable adults residing in low-income housing designated for older adults with a goal of helping them to "age in place" while improving quality of life through health equity. Through the promotion of wellness and care coordination among vulnerable older adults, RWHP will connect participants to resources that meet their health, nutritional, and safety needs and provide a meal to address nutritional needs and food insecurity.

Eligibility Inclusion Criteria:

- Resident of senior low income housing apartment buildings in Richmond, VA

Exclusion Criteria:

- Persons with cognitive impairment that are not able to engage in setting behavior goals

- Individuals with more than 1 of 3 positive on the Mini-Cog

Healthy Meals Program
RHWP provides a weekly meal along with promotion of wellness and care coordination among vulnerable older adults.

Other Change in Mobility The Timed Up and Go Test (TUG) is a simple test used to assess a person's mobility and requires both static and dynamic balance. It uses the time that a person takes to rise from a chair, walk three meters, turn around, walk back to the chair, and sit down. The TUG is used frequently in the elderly populations, as it is easy to administer in 10 minutes Baseline to 6 months
Other Change in Food Insecurity The United States Department of Agriculture (USDA) Short form is a 6-item form of the survey module and the associated 6-item Food Security Scale developed by the National Center for Health Statistics. This scale will be administered to ensure food insecurity needs have been addressed Baseline to 6 months
Primary Number of participants setting a health behavior goal related to nutrition Investigators will assess the individual's knowledge and skill related to wellness and nutrition goal by using the Patient Activation Measure (PAM) to determine the individual's readiness to establish a health behavior goal related to nutrition. Individuals who are determined to be ready based on their PAM score will be asked by the investigators to set a health behavior goal. 6 months
Primary Change in depression scores The Geriatric Depression Scale 15 will be used to assess the extent of depression. This is a 15 point scale with higher scores indicating greater depression. Baseline to 6 months
Secondary Change in Social Isolation The Lubben Social Network Scale records distinct levels of social interaction with relatives and friends. The scale measure the components of family networks, friend networks and interdependent social supports, and takes 5-10 minutes to complete. Higher scores indicate more social engagement. Baseline to 6 months
Secondary Change in Quality of Life The 12-item Short Form Health Survey (SF-12)asks for individual's views about their health. This information will help keep track of how they feel and how well they are able to do usual activities, with higher scores indicating lower quality of life. Baseline to 6 months
