To better understand the time course of functional walking recovery within the first 6 months post-stroke.

Clinical Trial Description

In this study, the researchers aim to investigate the time course of walking recovery in a repeated-measurement design. Through measuring recovery with impairment- and activity-scales, as well as lab-based analyses of standing/walking performance, they are able to delineate between the restitution of 'normal' (i.e. pre-stroke) movement patterns and the use of alternate strategies to compensate as mechanisms driving recovery within the first 6 months post-stroke.

Previous research found associations between improved standing/walking ability and asymmetric control strategies, as stroke survivors appear to heavily rely on the unaffected lower limb to keep an upright posture and move the body forward during walking. Obviously functional recovery after stroke is not only a matter of restitution, but also the development of such compensation strategies, while the latter appears to have a greater functional impact. The researchers want to add to this literature by identifying the temporal pattern of walking recovery and these underlying mechanisms. Similar observational studies on upper limb recovery have identified the first 5-8 weeks post-stroke to be critical for restitution while functional gains beyond this period are explained by compensation, e.g. reaching performance improved through the development of compensatory trunk movements.

Such longitudinal studies investigating the impact of behavioral changes on functional outcome through observing patients from the first days onwards are needed to understand the complexity of recovery and to design interventions specifically aiming to modulate those mechanisms early after stroke. ;
