Intestinal Metabolic Reprogramming as a Key Mechanism of Gastric Bypass in Humans

The purpose of this research study is to determine how gastric bypass surgery effects metabolism in obesity and Type 2 Diabetes. One mechanism that has been investigated in animal models is change to the biology of the small intestine (Roux limb) and how glucose and other fuels are metabolized (or how the body digests and uses sugar and other fuels). This study will evaluate the role of the intestine in the beneficial metabolic effects of gastric bypass surgery. It specifically will examine whether the intestine increases its metabolism and its activity, and whether this results in an increase in fuel utilization. Thirty two (32) subjects will be recruited (18 with and 14 without Type 2 Diabetes). At the time of gastric bypass surgery, a small piece of intestine that is usually discarded will be collected. At three time points over the first year after surgery, intestinal samples will be obtained by endoscopy or insertion of a lighted flexible tube through the mouth. Blood samples will be taken at all time points, as well. All samples will undergo comprehensive metabolic analyses. Comparisons will be made between the two groups to understand the metabolic changes over time and if there are differences between the two groups.

Incretin Effects of Branched Chain Amino Acids

Branched chain amino acids (BCAA) are known to exert insulinogenic effect. Whether this effect is mediated by incretins (GLP-1, GIP) is not known. The aim of the study was to show incretin effect of BCAA, i.e. whether the oral administration of BCAA elicits higher insulin and incretin response when compared with IV route of the same dose of BCAA. Eighteen healthy, male subjects participated in three tests: IV application of BCAA solution (30.7±1.1 g of BCAA, IV BCAA) second was oral ingestion of BCAA capsules in the same dose (ORAL BCAA) and third experiment was oral placebo (PLACEBO). Glucose, insulin, GLP-1, GIP, valine, leucine and isoleucine levels were measured at time interval for up to 4 h.

Chile Biliary Longitudinal Study

Background: The gallbladder is a small organ below the liver. Gallbladder cancer is a leading cause of cancer death among women in Chile. Chile also has among the highest rates of gallbladder cancer and death in the world. Researchers in a high-risk region of Chile want to find out more about this type of cancer. They especially want to know how inflammation affects the way it develops. Objective: To better understand the causes of gallbladder diseases and factors that may be associated with them. Eligibility: Women ages 50 74 with gallbladder cancer who are covered by the Chilean national health system. Other people over age 21 with gallbladder disease. Healthy adults over age 21. Design: Participants may be screened with an ultrasound. They may be asked questions about their health. Participants will have an ultrasound of the abdomen. This can be done at the study clinic or in the hospital or at home. The ultrasound uses sound waves to take pictures of the gallbladder. Participants will have a physical exam. Participants will have their mouths looked at for tooth loss, dentures, and gum redness. Participants will give a blood sample or saliva sample. Participants will be asked for access to their medical history. They may be asked to allow researchers to keep any samples taken during gallbladder surgery. Participants will answer questions about their medical history. This will include illnesses, surgeries, medicine use, and family history. It will also include diet and lifestyle habits, such as places where they have lived and worked. Some participants will have 3 follow-up visits over 6 years. ...

Evaluation of Potential Screening Tools for Metabolic Body Odor and Halitosis

This study is designed as a retrospective cohort study to evaluate the potential of diagnostic procedures in defining populations of patients self-reporting unexpected and uncontrollable episodes of body odor and/or halitosis. The cohort - generally healthy individuals who had underwent multiple diagnostic tests recommended by their physicians and had not been diagnosed with any known medical condition - expressed their interest in trying gastrointestinal and nutritional diagnostic tests offered by Biolab Medical Unit. Our retrospective analysis will determine if these tests were useful as potential screening tools for metabolic body odor and halitosis.
