Effects of Sativex(Registered Trademark) and Oral THC on Attention, Affect, Working Memory, Reversal Learning, Physiology and Brain Activation

Background: - The therapeutic modalities of cannabis have received more research attention recently with the discovery of its ability to stimulate appetite and to provide pain and nausea relief in patients with AIDS, cancer, and multiple sclerosis, among other diseases. Sativex(Registered Trademark), an experimental drug derived from the marijuana plant, contains tetrahydrocannabinol (THC) and cannabidiol (CBD), both of which affect brain activity. Sativex(Registered Trademark) is being tested to determine how and to what extent it affects brain activity. - Functional magnetic resonance imaging (fMRI) uses magnetic waves to study brain activity. Researchers are interested in using fMRI to study how Sativex(Registered Trademark) affects regional brain activity, including thinking abilities and behavior. Objectives: - To study changes in regional brain activity produced by Sativex(Registered Trademark) compared with THC and placebo. - To determine how Sativex(Registered Trademark) is processed by the body. Eligibility: - Individuals between 18 and 45 years of age who are either current users of cannabis (less than daily) or healthy volunteers who do not use cannabis. Design: - The study will involve one training session and five testing sessions on separate days. - At every session, subjects will receive either THC or placebo capsules and either Sativex(Registered Trademark) or placebo spray. - Participants will complete a training session in a mock fMRI scanner to adapt to the fMRI scanning environment. In the training session, participants will practice the tests that will track thinking ability, attention, working memory, and other cognitive tasks. - Participants will have five fMRI scanning sessions with the tests they have practiced previously, and will provide blood, urine, and saliva samples as required by the researchers. Risk Perception in Drug-Dependent Adults With and Without Schizophrenia

Background: - Several studies of risk perception have demonstrated a common bias known as unrealistic optimism, in which individuals feel they are less likely than other people to experience unpleasant or harmful events in their lives, but more likely to experience pleasant or beneficial events. - Previous research has indicated that individuals with schizophrenia have less of a sense of unrealistic optimism about adverse events than individuals without schizophrenia. However, research on risk perception in schizophrenia is sparse, primarily reporting on behaviors and decisions in the laboratory that likely are influenced by risk perception. - Risk perception among substance users may be viewed in two separate categories: perception of vulnerability to adverse events and perception of vulnerability to negative outcomes associated with substance use. Research in both areas has yielded mixed results. Researchers are interested in studying the connections among schizophrenia, addiction, and risk perception in order to develop better drug use prevention and treatment programs for people with and without schizophrenia. Objectives: - To compare unrealistic optimism bias in people with and without schizophrenia and/or drug dependence, and its association with actual risky behavior. Eligibility: - Individuals between 18 and 64 years of age who fall into one of the following study categories: - diagnoses of both drug dependence (marijuana or cocaine) and schizophrenia/schizoaffective disorder - diagnosis of drug dependence only (marijuana or cocaine) - diagnosis of schizophrenia/schizoaffective disorder only - healthy volunteers with no history of drug use or serious mental disorder Design: - The study will require a single visit to the research center for a 5- to 6-hour session. - Participants will complete questionnaires on medical and behavioral history, complete tests of thinking skills like memory and attention, complete a brief computerized decision-making task, and answer questions about risk perception. - Participants will also provide urine samples and breath carbon monoxide measurements to test for recent use of tobacco and other substances.

Pilot Study Repetitive Transcranial Magnetic Stimulation (rTMS) in Cannabis Craving

Background: - Cannabis, also known as marijuana, is a commonly abused drug. There is no approved medication to treat cannabis addiction. The desire to use cannabis often increases when seeing others use it or seeing pictures of it. Researchers are interested in determining which parts of the brain are active when looking at pictures associated with cannabis. - Repetitive transcranial magnetic stimulation (rTMS) uses magnetic pulses to stimulate the brain. These pulses can change activity in parts of the brain. Researchers are interested in determining whether rTMS can decrease activity in the parts of the brain that respond to cannabis, and thereby lessen cravings for cannabis. Objectives: - To determine whether transcranial magnetic stimulation can lower craving for cannabis when people who use cannabis are shown images that increase craving. Eligibility: - Individuals 18 years of age and older who are physically healthy and currently use cannabis (at least 3 times weekly for the past 2 years). Design: - The study will involve eight visits over 3 weeks, with each visit lasting 1 to 2 hours. - Participants will have an initial assessment about cannabis use and provide blood and urine samples before beginning the study. - Participants will have three magnetic resonance imaging (MRI) brain scans. The first MRI scan will look at the structure of the brain at rest. During the other two scans, participants will look at images related to and not related to cannabis use. - The rTMS sessions (with either actual TMS or inactive [sham] TMS) will be held daily for 5 consecutive days. During these testing visits, researchers will ask questions related to drug use and craving, and collect urine and breath samples for further study. - Participants will have two follow-up visits, 1 week and 2 weeks after the rTMS sessions, to evaluate memory and mood, and one final MRI brain scan at the end of the study.

A Trial of N-Acetylcysteine (an Over-the-Counter Medicine) in Adolescents Who Smoke Marijuana

This study is investigating how N-Acetylcysteine (NAC), an over-the-counter medication, will reduce marijuana use when combined with Contingency Management, a behavioral treatment. Marijuana is the most commonly used illicit drug in the United States. However, the treatment options for cannabis dependence are limited; notably, no effective pharmacotherapy has been developed. Conceptually, the ideal medication treatment for cannabis dependence would: 1. be safe when administered to patients actively using cannabis 2. reduce cannabis intake and promote abstinence 3. treat the symptoms of cannabis withdrawal 4. reduce craving and relapse risk 5. have a low abuse liability.
