The Effect of Melatonin on Early Signs of Hypertension in Teenagers With Diabetes Mellitus Type 1

Nocturnal hypertension is recognized via ambulatory blood pressure monitoring in adolescents with type 1 diabetes mellitus. Melatonin, (as previously seen in earlier studies in adults), may alter these changes, which may be a benefit especially for patients with diabetes mellitus who are at risk for cardiovascular changes. The purpose of this study is to estimate the use of melatonin as treatment for nocturnal hypertension in young adults with type 1 diabetes melitus.

A Randomized Controlled Trial of Sertraline in Paroxysmal Arterial Hypertension

There is currently no established treatment for paroxysmal hypertension, but selective serotonin reuptake inhibitors showed good effect in previous reports. This double-blind, placebo controlled, prospective multicenter clinical trial will assess the efficacy of sertraline on cessation or reduction of symptoms of paroxysmal arterial hypertension. 136 patients with documented hypertensive paroxysms with abrupt elevations of blood pressure and distressful physical symptoms will be randomized in a 1:1 ratio to receive sertraline, 50 mg daily, or matching placebo as an add-on to their chronic medication. Effect of the treatment on patient symptoms, office and ambulatory blood pressure and side effects will be evaluated after 3 months. If proven effective, sertraline might become a standard treatment for this condition.

The goals of our study are to determine a).the association between abnormal circadian BP and the development of paroxysmal AF in hypertensive patients, b).at which level of TOD, paroxysmal AF episodes are detected in hypertensive subjects, c).if there is any association between systolic and/or diastolic BP levels with AF occurrence, d).whether the mean heart rate during a 24-hr interval is associated with the development of paroxysmal AF, and finally e).examine the relationship between a wide PP and asymptomatic AF episodes in patients with HTN.

Equol Supplementation on Blood Pressure and Vascular Function

We propose to perform a 24-week randomized, double-blind a placebo-controlled trial among 207 non-equol producing postmenopausal women with prehypertension to examine the effectiveness of equol (10mg and 20mg/d) on 24h ambulatory blood pressure, vascular function and other cardiovascular risks (lipid profile, glycemic control and inflammatory biomarkers) and explore the optimal dosage of equol.
