Eligibility Inclusion Criteria: - Patients with chronic heart failure (CHF) failure who are prescribed with JARDIANCE® Tablets in Japan - Patients who have never been treated with Empagliflozin (including treatment for type 2 diabetes mellitus (T2DM)) before enrolment Exclusion Criteria: - None
