Inclusion Criteria: - Diagnosis of heart failure, discharged from any medical service Exclusion Criteria: - Unwilling to participate, discharged to nursing/rehab facility, non-English speaking, resident unavailable |
Allocation: Non-Randomized, Endpoint Classification: Safety Study, Intervention Model: Factorial Assignment, Masking: Open Label
