The purpose of this study is to demonstrate the feasibility of introducing, implementing, and integrating a brief, existent elder mistreatment screening tool (the National Collaboratory to Address Elder Mistreatment (NCAEM)'s Elder Mistreatment - Screening And Response Tool (EM-SART) Pre-Screen) in the Memorial Herman Hospital (MHH)-Texas Medical Center (TMC)Acute Care of Elders(ACE) (MHH-TMC ACE) unit and to identify older adults who may already be suffering from abuse, neglect, and/or financial exploitation, as well as connecting the study participants to the primary Geriatric Medicine team on the ACE unit such that additional care, evaluation (including additional in-depth mistreatment screening), and intervention can be taken to prevent and minimize further harm prior to discharge. The investigators hope to reduce missed opportunities for mistreatment detection to lead to safer health outcomes and hospital discharges.
The objective of this project is to pilot test an ADL (activities of daily living)-enhanced program as an adjuvant therapy to usual home health rehabilitation to improve patient outcomes. The project will compare the ADL-enhanced program plus usual care with usual care using an RCT design in home health patients.
INTRODUCTION: aging is the natural process of deterioration of the reserves of biological systems, human beings are inherent to this process. The elderly population in Brazil grows very quickly, this is due to the drop in fertility, mortality and increased life expectancy. OBJECTIVE: to promote the planning of guiding and educational actions based on the qualification of the frailty of the elderly in the Basic Health Unit of the study through the use of the Multidimensional Assessment of the Elderly in Primary Care (AMPI/AB) tool. METHODOLOGY: an experimental, longitudinal, prospective and analytical study aimed at approximately 20 pre-frail elderly people residing in a specific area of a Family Health Unit in the city of Guarulhos/SP. A survey of data referring to the application of AMPI/AB carried out in 2021 and classified as pre-fragile will be carried out, in order to later select them for research. The participant will receive educational guidance on nutrition, use of medications and nursing guidance as identified as needs. Three months after the intervention, AMPI/AB will be applied in order to observe whether the pre-frail elderly remained or changed category after the experimental multidisciplinary educational interventions.
ED PLUS Emergency Department Discharge Physiotherapy Led Community Service is a pilot feasibility randomized controlled trial investigating the role of an integrated care intervention consisting of comprehensive geriatrics assessment in older adults in the emergency department and a physiotherapy-led community based intervention.
Several studies have shown that going to the emergency room is a risk factor for loss of independence in the elderly. It has been shown that the period following an emergency room visit without hospitalization is a period of vulnerability for the elderly. The functional decline, or loss of functional autonomy, of the elderly is associated with an increase in institutionalization, mortality and costs to society. Studies have highlighted the risk factors for functional decline in the elderly, such as pre-existing functional and cognitive decline, undernutrition, but no model of care has yet prevented the risk of loss of autonomy after a stay in the emergency room. A full and early geriatric assessment could prevent functional decline after the emergency room visit. The primary objective of the study is to assess the impact of a dedicated geriatric sector on the functional decline at 1 month of patients admitted to emergencies without hospitalization by comparing an intervention group (patient having benefited from the geriatric sector) and a group witness ("classic" emergency patient). The secondary objective is to evaluate the impact of this sector on the number of falls at home as well as the readmission rate within 1 month of going to the emergency room.
The aim of this study is to design and assess the accuracy of administering the electronic Geriatric Assessment Bundle (e-GAB) compared to geriatrician performed comprehensive geriatric assessment (CGA) in a group of Egyptian elderly. The development of an Arabic software platform for CGA can improve the clinical practice by providing a valid user friendly tool to collect and analyze data for geriatric patients attending non- Geriatric health care service.
The present study is to examine the effects of a multicomponent exercise on executive function and balance in pre-frail older adults.
Study of interest of Telemedicine in establishments providing care for the dependent elderly (EHPAD in french) is a strategy for improving the course of care of residents, including the number not programmed hospitalizations, and a multidimensional analysis of the conditions of effectiveness of telemedicine will identify the keys to optimize its use.
Cohort Observational Study. The study will assess differences in quality of life, well-being and mood between persons living in a nursing home following usual care and persons living in nursing homes following a centered person care model. Initially, the follow up will last 6 months, with the possibility to be extended.
