Near Infra-red Spectroscopy for Detection of Intracranial Haematoma

Fall with head injury is becoming an epidemic challenge especially with the ageing population. Contributing factors for mortality and poor functional outcome included development of cerebral contusion and delayed traumatic intracerebral haematoma. There is a higher prevalence especially with the increasing use of antiplatelets and anticoagulants. Non-invasive monitoring such as near-infrared spectroscopy (NIRS) is sensitive in detecting intracranial changes. The role and efficacy of this non-invasive method has not been specifically established in patients with head injury as an initial non-operative monitoring. This is particular important in the setting of a general ward in which nursing staff is limited. The advantages of these noninvasive monitoring might have a role of continuous neuro-monitoring. They can also potentially reduce the number of unnecessary repeated CT Brain in the context of limited radiology staff and resources. Timely detection and treatment of this condition accordingly is crucial. Potential options of non-invasive monitoring such as nearinfrared spectroscopy (NIRS) is to be investigated. The aim of this study is to determine the sensitivity and specificity of NIRS as a non-invasive monitoring in detecting delayed intracranial injuries in comparison with the Gold Standard CT Brain. Study design is Prospective sensitivity and specificity study of Near Infra-red Spectroscopy (NIRS) as a non-invasive monitoring in detecting delayed intracranial injuries in comparison with the Gold Standard CT Brain in Hong Kong Chinese. Consecutive patients admitted to Prince of Wales Hospital, Hong Kong would be recruited. Outcome measures including correlation of non-invasive monitoring with near-infrared spectroscopy (NIRS) to CT Brain findings including any increase in haematoma size, cerebral edema or mass effect. Secondary outcome including 30 days mortality and functional outcome at 3 months.

Ving Tsun Martial Exercise for Older Adults

Objectives: To evaluate the effects of a Ving Tsun (VT) reactive balance training programme on reactive balance control, muscle strength, balance confidence and falls in community-dwelling older adults. Hypothesis: The VT group subjects will have improved post-training reactive balance control, muscle strength, balance confidence and fall incidence outcomes than the active controls. Design and subjects: In this prospective, randomised, single-blinded controlled trial, approximately 114 healthy seniors (55-70 years old) will be randomly assigned to either the VT group (n~57) or control group (n~57). Interventions: Subjects in the VT group will receive VT reactive balance training (3 hours/week) for 3 months, whereas subjects in the control group will receive no VT training but will jog 3 hours/week during the intervention period. Study instruments and outcomes: Primary outcome measures: reactive balance control, as indicated by lower extremity muscle activation onset latency; hip and ankle strategies and centre of pressure movements will be measured by electromyography, electrogoniometry and a force platform, respectively. Secondary outcome measures: knee muscle strength, balance confidence and fall history will be assessed by an isokinetic dynamometer, Activities-specific Balance Confidence Scale (Chinese) and interviews, respectively (pre-, post- and follow-up measurements). Data analysis: Data will be analysed using repeated-measures analysis of (co)variance followed by post-hoc tests, as appropriate (alpha = 0.05).

Mental Attention-neuromuscular Training for Children With Developmental Coordination Disorder

Objective: To compare the effectiveness of EEG biofeedback mental attention-neuromuscular training (AT-NMT), neuromuscular training (NMT) alone, EEG biofeedback mental attention training (AT) alone, and no intervention for improving reactive balance performance among children with developmental coordination disorder (DCD). Design: A single-blinded, randomized controlled clinical trial. Sample: 172 children with DCD. Interventions: AT-NMT, NMT, AT, or no intervention for 12 weeks. Major outcomes: Outcomes will be evaluated at baseline, post-intervention, and 3-month follow-up. A motor control test (MCT) will give a composite latency score, prefrontal cortex EEG recordings during MCT will measure the mental attention level, and surface electromyography recordings during MCT will indicate the lower limb muscle onset latency.

A Trial to Investigate the Impact of a Low Glycemic Index (GI) Diet on Body Mass Index and Obesity Related Cardiovascular and Hormonal Factors in Chinese Adolescents

Epidemiological studies suggest a role for a low glycemic index (GI) diet in the management of obesity and associated metabolic risks including diabetes. However, evidence from long-term, randomized controlled trials exploring the relationship between low GI diet, weight reduction and glycemia, particularly in children and adolescents, is lacking. Modern food-processing technology has produced many food products with high GI which may contribute to the burgeoning epidemic of obesity worldwide. Since dietary habits are shaped in early life, adolescence is a critical period to educate our young people to acquire a healthy eating habit to prevent obesity. The investigators hypothesized that, in Chinese adolescents, low GI diet results in greater reduction in body mass index and body fat percentage, enhanced insulin sensitivity and favorable changes of cardiometabolic risk factors compared to conventional diet after 12 months of intervention. This study is a randomized controlled trial of a low GI (<55) versus conventional Chinese diet (GI>/=70) in adolescents (12-month intervention followed by a 6-month observational period) to study; 1) the changes in body mass index and obesity associated changes in cardiometabolic profile; 2) the underlying hormonal factors associated with these changes.

Effects of Vitamin C and Lycium Barbarum Supplements on the Progression of Senile Cataract in Hong Kong Chinese Population

With the rapid growing aging population, age-related eye diseases including cataract are becoming more prevalent. The demand for cataract surgery and post-surgical care posed enormous financial burdens to the government. Vitamin C, also known as ascorbate, is a highly effective antioxidant which may help to protect the cells including the crystalline lens components from oxidative damage, which is believed to be one of the causes of senile cataract. Lycium barbarum, on the other hand, contains lutein and zeaxanthin which can filter the phototoxic blue light and neutralize the reactive oxygen species, thus provide a protective effect against cataract formation. This study aims to determine the effects of Vitamin C and Lycium barbarum Supplements on the progression of senile cataract in Hong Kong Chinese elderly.
