Sex, Psychopharmacology, and Diabetes

The term sexual (SD) dysfunction covers conditions that prevent people from having a satisfactory sex life. SD is a frequent and sometimes debilitating complication of mental illness and a known adverse reaction to psycho-pharmacological treatment. SD is also associated with diabetes, a common somatic comorbidity in psychiatric patients. SD is associated with both reduced quality-of-life and reduced treatment adherence, yet SD is far too rarely addressed between the patient and the healthcare professional in clinical consultations. The purpose of the study is to investigate whether targeted education of patients with schizophrenia and diabetes/prediabetes and/or their healthcare professionals in causes and management of SD: - Increases the number of systematic examinations of sexual side effects, - Causes changes in the psycho-pharmacological treatment, and - Reduces the severity or perception of sexual side effects. The study is a multicenter Randomized Controlled Trial (RCT) with four arms, in which the educational intervention is provided to patients, healthcare professionals, or both groups. The effect of the educational intervention is compared to a non-educated control group. The study is expected to include 192 patients recruited from 16 assertive community treatment centers evenly distributed in four Danish regions. The study is part of an interdisciplinary project named SECRET. The educational intervention was developed in an ethnographic pre-study incorporating stakeholder engagement. Parallel to the present RCT, an ethnographic field study will be carried out to broaden the perspective on the effects of the intervention.

Experience of Horticultural Group Therapy Among People With Chronic Schizophrenia

Taiwan National Health Insurance Administration database shows that the number of people seeking medical treatment due to mental illness has reached 1.454 million in 2021. The annual growth rate of mental illness is between 3.3% to 6.6%. Fifty percent of mental illness patient has catastrophic illness certificate because of chronic schizophrenia (Ministry of Health and Welfare,2022). Auditory hallucinations, delusions, emotional disturbances, and disorder in behavior and the poor ability of speaking are common in patients with schizophrenia. These symptoms lead to reduce the cognitive and executive function, resulting severe impairment of eventual daily living functions and social interactions. Thus, the cost of hospitalization and medical fee is increased. It is worth considering the appropriateness of the model of care. According to researches, people with schizophrenia still strive to pursue a happy and satisfying life. However, statistics of satisfaction and happiness are still lower than people who are healthy (Palmer et al., 2014; Fervaha et al., 2016). The American Horticultural Therapy Association (AHTA) pointed out that horticultural therapy increases the well-being of participants through active or passive participation in plant-related activities. It also helps participants learn new skills or regain lost skills. Furthermore, psychiatric symptoms, recovery outcomes, life satisfaction, and the benefits of social function are significantly improved (Lu et al., 2021; Oh et al., 2018). Taiwanese scholars mention that combining horticultural activities and group psychotherapy improves the therapeutic results. It means that using of flowers and plants in gardening combines with the therapeutic advantages of group psychotherapy, mental illness patients can benefit more from it (Lin, 2016). This study aims to explore the experience of patients with chronic schizophrenia participating in horticultural group therapy. This research design adopts the purposive sampling in qualitative method from a psychiatric hospital in central Taiwan. It is expected that 10 participants will involve in 12 times of horticultural therapy, and recordings will be made in each group. After 12 times of the therapy, participants will be involved in semi-structured audio-recorded interviews. The collected data were analyzed step by step using the content analysis method. In addition, participants will finish the questionnaires of the "Mental Well-Being Scale"," Satisfaction With Life Scale ", " activity of daily living Scale ", Instrumental Activities of Daily Living Scale", and "Group Efficacy Observation Form" before and after the therapies. The data will be conducted with descriptive statistics, Pearson's correlation coefficient, Chi-square Test, Paired Sample t test by using SPSS 21.0 statistical software. This research hopes to understand the impact of horticultural treatments in people who suffer from chronic schizophrenia, and provides a reference to clinical teams.

Neurophysiologic Biomarkers for Cognitive Rehabilitation

Cognitive symptoms of schizophrenia interfere with daily life-from managing self-care, to more complex tasks like taking medications and living independently. Unfortunately, these cognitive symptoms are not corrected by 'standard of care' treatments (antipsychotic medications), although some schizophrenia patients may experience modest clinical and cognitive benefits from cognitive remediation. To enhance the clinical impact of cognitive remediation and other rehabilitative interventions for Veterans living with chronic psychosis, this study will develop novel brain-based tools to help identify those Veterans who are most likely to benefit from pro-cognitive therapies. These studies may advance predictive algorithms that improve functional outcomes and life quality in Veterans with schizophrenia.

Critical Time Intervention-Peer Support

There is increasing awareness of the importance of providing mental health services and support that promote a recovery-oriented and human rights-based approach. A mental health service system that is guided by a rehabilitation and recovery perspective places emphasis on treating the consequences of the illness rather than just the illness "per se", and on empowering people to regain control of their identity and life, and to have hope for the future. Within this philosophy, mental health policies in several countries advocate for the introduction of peer workers in mental health services, people with lived experience of mental health issues and recovery, who are employed to use their lived experience to support those who access mental health services. However, more effectiveness and implementation research is needed. Evidence also suggests that the period following hospital discharge is of high risk of treatment dropout for people with serious mental illness, thus interrupting their recovery process. Therefore, this vulnerable population may particularly benefit from more targeted interventions during this transitional period. The research project will conduct a randomized controlled trial to evaluate the effectiveness, feasibility and implementation of the Critical Time Intervention-Peer Support model, a recovery-oriented based model for people with serious mental illness discharged from inpatient psychiatric treatment facilities in Portugal. The randomized controlled trial (RCT) will be conducted in three psychiatric services in the Lisbon Metropolitan Area and their catchment areas. People with diagnoses of psychotic disorders discharged from inpatient psychiatric treatment facilities will be recruited and randomly divided into CTI-PS intervention or usual care. Those allocated to the intervention group will additionally receive CTI-PS rather than usual care alone over a 9-month period. Outcomes at baseline, 9- and 18-months will be analyzed by multilevel models, considering the observations clustered within sites. Longitudinal analyses will be used to examine trends over time of the outcomes of interest. The implementation of the CTI-PS model will introduce a novel approach to community mental health care that has not yet been tried in Portugal. This study aims to explore to what extent this intervention can be effective and implemented in countries with the characteristics of Portugal. Additionally, the proposed research aims to contribute to the global knowledge about peer interventions by investigating whether the CTI model maintains its effectiveness using peers.

Dextromethorphan as an Augmentation Agent in Treatment-resistant Schizophrenia

Dextromethorphan acts as N-methyl-D-aspartate (NMDA) antagonist. In Treatment resistant schizophrenia(TRS) the efficacy of treatment response by clozapine is only around 40%. Numerous augmentation agent have been tried which includes antipsychotics, anticonvulsants, antidepressants and NMDA antagonist. The NMDA antagonist such as Riluzole and Memantine have shown good efficacy in TRS. Therefore we are evaluating NMDA antagonist, dextromethorphan in TRS. The dextromethorphan or placebo will be administered along with clozapine in TRS patients. The study is randomized double blind placebo controlled group sequential trial.

Effectiveness of Exergames on Cognitive, Social Functionamong Chronic Schizophrenia

An experimental study with random assignment will be adopted. The participants from 2 hospitals will be assigned to two groups: experimental group (n=77), and control group (n=77). Experimental group, in addition to scheduled activities, will be provided with the "individual brain training exergames (a program for improving cognitive function and eye-hand coordination)" three times a week and 30 minutes each time. The control group will be maintained with their scheduled activities in daycare as usual. Behavioral intentions, cognitive function, social function, and hand-eye coordination will be measured at 0, 3, 6, 9, 12 weeks.

Song-making In a Group (SING)

The overarching aim of the proposed work is to align a promising treatment lead - Musical Intervention (MI) - with a promising mechanistic account of psychosis - Predictive Processing. This protocol focuses on the R33 phase, to optimize its administration (Is active participation more effective than passive listening? Does creation of new music help more than performing others' creations?). By tracking the interrelation between symptom mechanisms and MI, the investigators can use those metrics to prospectively assign patients to particular MI. The R33 phase will examine the impact of SING on computational behavioral metrics of (Aim 1) Conditioned Hallucinations, (Aim 2) Social Reinforcement Learning, (Aim 3) Language Use, in 200 participants with voice hearing in the context of a psychotic illness (n=50per per group). Following a screening visit to determine eligibility, these computerized tasks will be administered behaviorally, and an interview will elicit speech, prior to and following the full SING intervention (in 10 groups of 5 participants, each facilitated by a trained musical interventionist, during the first two years of the project). Participants will complete these tasks prior to and following randomization to four different conditions (facilitated by a SING team member) that will deconstruct the possible active ingredients of SING along two dimensions: Activity and Ownership: (a) SING (n=50, Activity + and Ownership +), participants produce and perform their own song; (b) Karaoke (n=50, Activity + and Ownership -), participants perform karaoke, singing along to others music; (c) Pop Music (n=50, Activity - and Ownership -), participants will listen to popular music chosen by the music interventionists; and (d) Curated Playlists (n=50, Activity -, Ownership +), participants will curate playlists of popular music and listen to them together. This deconstruction will provide insights into the predictive processing framework, as applied to hallucinations and music, specifically, whether changes at higher, a-modal, hierarchical levels, particularly sense of self and active inference, influence precision weighted perceptual and social inferences more so than inactive experiences or experiences that do not engage sense of self. This R33 portion of the study was originally included in NCT05537428, which now has results posted for the R61 phase of the study.
