Transcriptome, Proteome and Microbiome Profile in Periodontal and Peri-implant Diseases

1. To investigate the transcriptomic profile of periodontitis and peri-implantitis. Patients with defined periodontitis, at two non-adjacent teeth and peri-implantitis at ≥ one implant in function of ≥ one year, will be included to investigate the gene expression profile in tissue affected by periodontitis and peri-implantitis. Subjects will undergo a full-mouth examination performed by a calibrated examiner, including assessment of caries, bone abnormalities and infections according to intraoral x-ray, plaque index and pocket probing depth, suppuration, bleeding on probing, alveolar bone loss, and the number of teeth. During surgical intervention, tissue biopsies (two specimens per site) will be collected by a periodontist from site with ongoing periodontitis and site from ongoing peri-implantitis. 2. To study the microbiome and biomarker profile associated with periodontitis and peri- implantitis. Patients with defined periodontitis, at two non-adjacent teeth and peri-implantitis at ≥ one implant in function of ≥ one year, will be included in this study to determine the bacteria composition, cytokine profile and inflammatory biomarkers profile. Subjects will undergo a full-mouth examination performed by a calibrated examiner, including assessment of caries, bone abnormalities and infections according to intraoral x-ray, plaque index and pocket probing depth, suppuration, bleeding on probing, alveolar bone loss, and number of teeth. Peri-implant crevicular fluid (PICF), gingival crevicular fluid (GCF), saliva and submucosal/subgingival plaque will be collected.The presence and composition of periodontal and peri-implant plaque samples are investigated.

Lingual Keratinized Tissue in Fully Edentulous Patients and Its Impact on Implant Health

The width of KT will be correlated with the primary outcome, diagnosis of peri-implantitis, defined as Probing depth (PPD)>= 6 mm, inflammation/ bleeding on probing (BOP) and bone loss >= 3 mm from the implant platform.

The Effect of Smoking on Peri-implantitis

the primary aim of this project is to evaluate the microbiological and inflammatory effect of smoking status and smoking severity on periimplantitis lesions. The secondary aim is to compare the effect of smoking on periimplantitis and periodontal microbiota and inflammation in the same individuals. There will include 96 patients, equally divided into four groups: Smokers with peri-implantitis (n=24), non-smoker individuals with peri-implantitis (n=24), smokers with healthy peri-implant tissues (n=24), non-smoker individuals with healthy peri-implant tissues (n=24). Microbiological and biochemical analyses will be performed on the samples taken.

Salivary Minerals in Patients With Peri-implantitis

Patients included in the study will be recruited after arriving at the Faculty of Dentistry of the University of Zagreb. During the clinical examination, the patient's periodontal status (bleeding during probing, and the depth of the pockets around the implant) and a control x-ray will be taken to check bone loss. Saliva sampling will occur at 3-time points, when establishing the diagnosis of periimplantitis, before the start of therapy, and at the first control after treatment.

Peri-implantitis and MMP-8

Peri-implantitis is defined as the pathological condition around dental implants characterized by inflammation in the peri-implant mucosa and progressive bone loss, eventually leading to implant loss. Peri-implantitis is thought to be a disease analogous to periodontitis with a prevalence reaching 22%. Though peri-implantitis is readily recognized as a part of modern dentistry, the exact etiology or an effective treatment regimen hasn't been established yet. Thus, contemporary research is orientating toward acknowledging the aetiologic and risk factors of the disease and of course establishing prognostic markers for disease prevention. Microbiota residing in the subgingival plaque are considered the main etiologic factor of the disease, however, current literature has not concluded on the exact microbial composition of peri-implant lesions. In addition, genetic predisposition has been recognized as a risk factor for disease initiation and progression and several observational studies have addressed the potential association between various gene polymorphisms and the occurrence of peri-implantitis. Lastly, to establish effective preventive measures, several biomarkers have been evaluated as potential diagnostic and prognostic markers of disease progression. Objectives: 1. To identify the relationship of peri-implantitis with Cycloxygenase-2 (COX-2) and MMP-8 gene polymorphisms. Cyclooxygenase catalyzes the production of prostaglandins (PGs) which are an important inflammatory mediator participating in the pathogenesis of peri-implantitis. In addition, PGE2 expression in the peri-implant crevicular fluid will be assessed. 2. To characterize the microbiota associated with peri-implantitis lesions, using novel identification techniques enabling the identification of specific opportunistic bacteria associated with the disease. 3. To test the diagnostic accuracy of a modern chairside test, using metalloproteinase-8 (MMP-8), an enzyme implicated in the pathogenesis of the disease, as a biomarker of disease progression.

Citric Acid Passivation on Implant Abutments for the Prevention of Peri-implant Disease

The use of dental implants is a predictable procedure to replace the missing tooth with high rates of success and survival. In addition, placing the abutment on the same day as the implant surgery is a safe and biologically sound procedure to minimize bone loss and peri-implant inflammation. Although citric acid antibacterial coating has never been tested in vivo, recent in vitro data support its clinical use due to its antibacterial effect at the abutment level. This prospective randomized controlled clinical trial aims to evaluate the microbiological and inflammatory profile of implant abutments coated with citric acid.

Adjunctive Benefit of Electrolytic Cleaning on Non-surgical Treatment of Peri-implantitis.

Evaluate the outcomes of non-surgical therapy of peri-implantitis with the adjunctive use of electrolytic cleaning (Galvo Surge®). The proposed protocol, focused on the non-surgical treatment of peri-implantitis, will result in a higher percentage of subjects with disease resolution (probing depts ≤ 5mm, absence of BOP and/or SOP and no further bone loss) at 1 year of follow-up.
