Eligibility Inclusion Criteria: - Patients attributed to the New York University Langone Health Clinically Integrated Network and have Medicare or Medicaid coverage (via Healthfirst) who are due for gaps in care Exclusion Criteria: - Any patients who do not meet the inclusion criteria
