Multimodal Deep Learning for the Diagnosis and Assessment of Alzheimer's Disease

Alzheimer's disease (AD) is the most common dementia and has been one of the most expensive diseases with the highest lethality. With the rapid increase of the aging population, more and more burdens will be posed on society and economics. The manifestations of AD are the progressive loss of memory, language and visuospatial function, executive and daily living abilities, and so forth. The Pathophysiological changes of AD occur 10-20 years before the clinical symptoms, while there is still a lack of effective strategy for early diagnosis. Mild cognitive impairment (MCI) is considered to be a transitional state between healthy aging and the clinical diagnosis of dementia and has received increasing attention as a separate diagnostic entity. To make the diagnosis, doctors ought to compressively consider the multimodal medical information including clinical symptoms, neuroimages, neuropsychological tests, laboratory examinations, etc. Multimodal deep learning has risen to this challenge, which could integrate the various modalities of biological information and capture the relationships among them contributing to higher accuracy and efficiency. It has been widely applied in imaging, tumor pathology, genomics, etc. Recently, the studies on AD based on deep learning still mainly focused on multimodal neuroimaging, while multimodal medical information requires comprehensive integration and intellectual analysis. Moreover, studies reveal that some imperceptible symptoms in MCI and the early stage of AD may also play an effective role in diagnosis and assessment, such as gait disorder, facial expression identification dysfunction, and speech and language impairment. However, doctors could hardly detect the slight and complex changes, which could rely on the full mining of the video and audio information by multimodal deep learning. In conclusion, we aim to explore the features of gait disorder, facial expression identification dysfunction, and speech and language impairment in MCI and AD, and analyze their diagnostic efficiency. We would identify the different degrees of dependency on multimodal medical information in diagnosis and finally build an optimal multimodal diagnostic method utilizing the most convenient and economical information. Besides, based on follow-up observations on the changes in multimodal medical information with the progress of AD and MCI, we expect to establish an effective and convenient diagnostic strategy.

The Mito-Frail Trial: Effects of MitoQ on Vasodilation, Mobility and Cognitive Performance in Frail Older Adults

The goal of this clinical trial is to test the effects of MitoQ supplementation in older adults and frail older adults with physical dysfunction and/or cognitive dysfunction. The main question[s] it aims to answer are: - To compare vascular function, oxidative stress levels, and physical and cognitive function among older adults and frail older adults with physical and cognitive dysfunction - To determine whether MitoQ supplementation has the potential to improve vascular function in central and cerebral vessels - To determine whether MitoQ supplementation can enhance physical and cognitive capabilities.

A rTMS and Virtual Reality Based Cognitive Rehabilitation Program for MCI

This clinical trial is being conducted to see if brain stimulation and brain training together improves cognitive functioning and mood in older adults diagnosed with Mild Cognitive Impairment (MCI). Brain stimulation will be done using repetitive Transcranial Magnetic Stimulation (rTMS). Brain training will be done using immersive virtual reality cognitive training (iVCT) program. The goals of this clinical trail are as follows: - Examine if rTMS+iVCT intervention can improve and sustain objective cognitive functioning in individuals with MCI more than control or rTMS only groups - Examine if rTMS+iVCT intervention improves participants mental health symptoms, functional abilities, and quality of life more than control or rTMS only groups - Examine the impact of rTMS+iVCT intervention on caregiver burden. Eligible participants will be assigned to a standard treatment (no intervention control) group, rTMS only group of rTMS+iVCT group. All participants will undergo baseline assessment to evaluate their cognitive, emotional, and functional abilities. Those in the rTMS only group will receive rTMS treatments for five days per week for two weeks (total of ten sessions). Those in the rTMS+iVCT group will receive rTMS treatment followed by iVCT training for five days a week for two weeks (total of ten sessions). All participants will then repeat testing 2 weeks and three months after baseline testing to assess for possible treatment related changes and lasting effects.

Light and Sleep Fragmentation

Memory Support System Feasibility Study

The purpose of this study is to determining the feasibility of providing the Memory Support System (MSS) to individuals with mild cognitive impairment (MCI) and their partners at a clinic in Ontario, Canada. This will involve a) collecting information from patients referred to the a memory clinic and geriatric day hospital about their interest in and their preferred method to administer the MSS; and b) a cost analysis related to implementation of the MSS. The study will also measure efficacy outcomes of the MSS regarding program adherence as well as to self-reported IADLs, self-efficacy for memory, quality of life, mood, anxiety, and caregiver burden among a sample of individuals with MCI and their care partners

Clinical Trial of AGB101 for Mild Cognitive Impairment

The primary objective of the study is to evaluate the efficacy of AGB101 on slowing cognitive and functional impairment as measured by reduction in neuronal injury in participants with mild cognitive impairment due to Alzheimer's Disease. Participants will be randomized to receive placebo or AGB101 (220 mg), once daily for 78 weeks. Secondary objectives are to assess the effect of AGB101 compared with placebo on clinical progression as measured by the Clinical Dementia Rating Scale- Sum of Boxes and Memory Box score.

Effect of Menopause Hormone Therapy In Postmenopausal Women With CSVD And MCI

The objective of this clinical trial is to explore the efficacy of menopausal hormone therapy in early menopausal women with CSVD and MCI. The main questions it aims to answer are: - The efficacy of menopausal hormone mainly estrogen therapy for early menopausal women with CSVD and MCI - The role of MHT in delaying the progression of cognitive function, CSVD imaging features, and other clinical symptoms and the potential pathophysiological mechanisms. Participants will be divided randomly into two groups taking MHT drugs and placebo respectively and followed up for 12 months to collect relevant clinical data.

LEADing Dementia End-of-Life Planning Conversations

Advance care planning is important for all adults, but perhaps even more so for the 5.7 million persons with Alzheimer's disease or related dementia (ADRD), due to the progressive and protracted cognitive deterioration associated with the disease process. In the context of ADRD, medical decision-making at the end of life is typically left to one's care partner, who often does not have the knowledge or confidence in their ability to make such decisions. This study will refine and evaluate a web-based platform, called the LEAD Intervention (Life-Planning in Early Alzheimer's and other Dementias), which is designed to help persons in the preclinical or early stage of ADRD engage in conversations about, document, and share their end-of-life values and preferences with a care partner, extended family members, and health care providers.

Individual Closed-Loop Neuromodulation Therapy for Alzheimer's Disease

The project is a placebo-controlled study that aims to use closed-loop transcranial alternating current stimulation (tACS) to study patients with symptoms of mild cognitive impairment which is likely due to Alzheimer's disease or another form of dementia (AD-MCI). Patients will undergo an EEG and complete some questionnaires and computer tasks during each study visit. The project has the following aims and hypotheses: 1.) To determine the impact of closed-loop 40 Hz tACS on the entrainment of natural gamma rhythms in patients with AD-MCI, 2.) To determine the impact of closed-loop 40 Hz tACS on cognitive performance in patients with AD-MCI, and 3.) To assess the relationship between baseline neurodegenerative burden and impact of tACS. Intervention Effect of Yizhi Baduanjin on Patients With Mild Cognitive Impairment

With mild cognitive impairment (MCI) as the research object, the intervention effect of Yizhi Baduanjin on mild cognitive impairment was evaluated by using the Montreal Cognitive Assessment (MoCA) scale, human-computer interactive electronic cognitive function score, multi-measure EEG data and other evaluation indicators, with the intervention measures of Yizhi Baduanjin created by the Tai Chi Health Center in the early stage. This project is based on the preliminary study, followed the principles and methods of clinical epidemiology and evidence-based medicine, and designed a prospective, randomized, single-blind, parallel controlled trial to make up for the deficiency of existing drugs in the intervention of mild cognitive impairment, and to provide evidence for the clinical and community promotion and application of effective and safe traditional Chinese medicine non-pharmaceuticals in the prevention and treatment of mild cognitive impairment.
