Emotional Regulation in People With Co-occurring Posttraumatic Stress Disorder and Substance Use Disorder

The aim of this project is to look at emotional regulation in people with posttraumatic stress disorder (PTSD) and substance use disorder (SUD). This study will explore how people with PTSD-SUD regulate their emotions and how this might explain the relationship between these two disorders. In turn, this may inform effective treatment strategies for people with comorbid PTSD-SUD. Emotional regulation refers to the way in which people process and respond to their emotions. PTSD and SUD commonly cooccur and this is associated with adverse outcomes including high rates of relapse, overdose, and suicide. We therefore need effective treatments to address this clinical concern. Evidence suggests emotional regulation might be important in the development and maintenance of PTSD and SUD and therefore it might be a useful target for treatment. However, most research in this area has been quantitative and has not considered how gender, social circumstances and trauma or substance type might affect the way people regulate their emotions. This study will recruit 40 adults with trauma histories and PTSD who are currently receiving treatment in a community drug and alcohol service for their substance use. Participants will be interviewed to explore how they regulate their emotions and how this relates to their social circumstances. This study will also explore whether gender, substance or trauma type affect the way people regulate their emotions. We hope this will help to improve treatment for people with PTSD and SUD.

An Intervention to Enhance Well-Being in Trauma Exposed New Mothers

This pilot randomized controlled trial will evaluate the feasibility and effectiveness of a mobile neurofeedback intervention for increasing maternal overall well-being, and measuring whether mothers experience any subsequent reductions in trauma symptoms and parenting stress and enhancements in regard to emotional regulation, parenting sensitivity and positive parenting behaviors, as well as infant socio-emotional development and behavioral outcomes (i.e., crying, fussing) among postpartum mothers with symptoms of post-traumatic stress disorder. The investigators hypothesize that mothers who receive the neurofeedback intervention will demonstrate larger decreases in mental health symptoms, greater improvements in emotional regulation and observed parenting behaviors, increased feelings of parenting competency, decreased feelings of parenting stress, and reductions in the potential for child maltreatment than mothers in the control group. The investigators also hypothesize that infants of mothers who receive the neurofeedback intervention will demonstrate less crying and fussiness and higher scores on socio-emotional developmental assessments than infants of mothers in the control group at the posttest interval.

Effects of School-based Yoga Program on University Students

Focusing on emotions is valuable because "how a person feels, reacts, and expresses emotions can have both short-term and long-term effects on physical and mental health". This is explained by mechanisms such as reappraisal, attention regulation, self-monitoring, self-awareness, and regulation of the autonomic nervous system. Because yoga reduces negative emotions such as anxiety, anger, and depression, teens are likely to result in less conflict and stress in their lives. It is thought that it is also important for young people to accept difficult feelings and to be able to accept and approve these feelings. When negative emotions are acknowledged and witnessed, they often dissolve or transform, and the process allows the individual to learn about their limits, preferences, and needs. Processing emotions in this way allows a person to be honest with oneself and can contribute to healthier development. Therefore, yoga appears to be a useful well-being tool and practice that schools should adopt, as it can increase life skills for students such as concentration, memory, relational skills, and decision-making that are affected by emotions. Emotional well-being is important for learning in life and school. As noted earlier, research supports such a view, but more research is needed to understand how and why yoga should be offered to young people in their schools. However, it is suggested that researchers further explore the role of yoga in the management of emotions, both in terms of emotional processing and regulation of emotions. The role of yogic breathing (pranayama) as part of a holistic perspective on yoga, and specifically the role of yoga in the relationship between being with emotions, regulating emotions, and how it relates to change, should be further explored. It has been described in the literature that care should be taken to avoid possible harm to individuals associated with the use of unhealthy weight control behaviors among young adults and women with obesity. Yoga's intent to strengthen and support a positive sense of self makes it a particularly viable strategy for healthy weight management for women and those at high risk for poor body image. Finally, it has been reported that reductions in perceived stress may mediate the effects of participation in a yoga program on negative emotional and behavioral problems. It has been suggested that future studies may also assess the extent to which exposure to stress and trauma may affect youth's participation in and benefit from mindfulness and yoga interventions. Considering all the suggestions and research needs in the literature, this study was planned to examine the improvement in self-esteem, life satisfaction, body image, anxiety, depression and cognitive emotion regulation levels of university students after their participation in the yoga program provided to them in the school environment and to compare them with students who do not do yoga. In the study, it is planned to investigate the pre-exam anxiety levels of university youth who regularly practice yoga.

Integration of DBT Skills and Parent Training for Parents With a History of Substance Use

This treatment development study is aimed at developing and pilot testing a 20-week remotely delivered group intervention that integrates two evidence-based treatments: 1) Dialectical Behavior Therapy (DBT) Skills - targeting parental emotion dysregulation and substance use, and 2) Parent Training (PT) - targeting parenting behaviors linked to children's mental health. The case study will provide preliminary data on the feasibility of the DBT Skills +PT intervention and its engagement of the targeted outcomes, and aims to complete foundational steps necessary to conducting a future, larger scale randomized controlled trial. Specific aims are to: 1. Determine feasibility, acceptability and implementation. 2. Evaluate pre-post and weekly changes in measures of parental emotion regulation, parenting stress, parenting quality and children's mental health.

Examining the Impacts on In-School Behaviors of In-Home Videogame Play

The proposed study aims to investigate the effects of children's in-home engagement with a biofeedback video game-based intervention on children's in-school behaviors. For the proposed study, families with no prior Mightier exposure will engage in online gameplay for eight weeks. Mightier will recruit families through social media and clinical settings, then Mightier will randomly assign participants to one of two groups: a group in which the child participant is encouraged to play Mightier games with biofeedback elements 45 or more minutes a week, broken into 3 or more sessions a week soon after baseline data have been collected, and a group in which the child participant receives Mightier and is invited to play Mightier games with biofeedback elements after all study baseline and follow-up data have been collected. The investigators will aim to enroll 20 parent-child dyads in each group. Specific Aims and Hypotheses: 1. Demonstrate that child engagement in Mightier biofeedback video games is correlated to child improvement in school behaviors. The investigators hypothesize that teachers of child participants will report reductions in child school behavior challenges after 8 weeks of child Mightier gameplay. 2. Demonstrate that child engagement in Mightier biofeedback video games is correlated to child improvement in at-home behaviors and general improvement in emotion regulation. The investigators hypothesize that parents of child participants will report a reduction in child behavior challenges and improvements in child emotion regulation after 8 weeks of child Mightier gameplay.

DISCOVER Workshop Programme for Care Experienced Young People

The NHS DISCOVER team has secured 2-year funding from the Maudsley Charity to tailor the workshop programme the investigators deliver in school Sixth Forms to meet the specific needs of Care Experienced young people. The investigators are planning to complete a pilot of this adapted workshop programme within Medway Children's Social Services to explore its feasibility, accessibility, acceptability and early-stage efficacy within this population of adolescents. Investigators from the DISCOVER team will recruit young people aged 16-19yrs who are currently known to the Medway Care Leavers 16+ teams. The study will be conducted at Medway Local Authority sites, with the option of some stages being completed remotely via Microsoft Teams if needed in the context of the ongoing COVID-19 pandemic.

Online Intervention for Traumatic Brain Injury Wellbeing

Traumatic brain injury (TBI) patients face notable impairments which lead to reduced performance and regulation of daily and overall functioning. There are a number of interventions made to combat these qualms; however, such interventions have historically been therapeutically demanding, which limits their practical benefit. An online therapeutic intervention can provide a cost-effective approach that can be particularly well-suited to the needs and limitations of TBI. It focuses both on developing awareness and attention, which are often impaired, and are critical to improving emotional and behavioral regulation and everyday function. This project is aimed at assessing the effectiveness and underlying mechanism of modified mindfulness based stress reduction (MBSR) using a rigorous randomized controlled trial. Poised to provide a rigorous approach to efficacy development and analysis, Results of the study will provide valuable information that will ultimately support the refinement of an intervention that can have a real impact on patients' ability to resume a fully functional and satisfying life, and the design of an adequate therapeutic intervention for TBI patients.
