Acute respiratory distress syndrome remains a deadly disease with hospital mortality remaining between 40 to 50%. ARDS mortality risk factors have been identified from patient history, common clinical and biological variables in the lung SAFE study. Part of ARDS mortality is attributable to ventilator-induced lung injury (VILI), in relation with inappropriate settings on the ventilator. Tidal hyperinflation and recruitment/derecruitment during lung inflation are 2 identified mechanisms leading to VILI, that may be identified on computed tomography while poorly identified with variables collected at the bedside. The aim of this study is to identify whether tidal hyperinflation identified on computed tomography is a risk factor for ARDS mortality, independently from know bio-clinical risk factors.
A bibliographic search carried out by the investigators demonstrated that there are few studies or data relating to the role and place of a State Registered Nurse (IDE) in a Mobile Palliative Care Team (EMSP) concerning the collection and the analysis of the needs expressed when formulating an evaluation request by the EMSP. The IDE collects, questions, analyzes needs and initiates an evaluation with the patient or, failing that, advises and/or redirects. This first necessary step before the overall evaluation led the investigators to question the autonomy of the IDE in an EMSP and to research what are the representations of an IDE in this role of collecting and analyzing needs. when making such a request? The aim of this research is to explore the professional practices of nurses within other PMSCs in order to analyze them and move towards improving patient care.
Modification of the rendering of the antibiogram at the Strasbourg University Hospital in November 2019 with the appearance of the concept of "standard dose" or "high dose" sensitivity. This modification seems to have favored an inappropriate overprescription of Meropenem (the only antibiotic made "at standard dose") in Pseudomonas infections sensitive to other beta-lactams. In June 2021, it was therefore decided to mask sensitivity to carbapenems by default in the rendering of Pseudomonas antibiograms when the strain was sensitive to a narrower spectrum beta-lactam ("restricted" antibiogram). The aim of this study is to evaluate the impact of these changes in the antibiogram on antibiotic prescriptions.
Desmoid tumors, also called aggressive fibromatosis, are rare, locally invasive tumors with no potential for metastasis. The incidence is approximately 2 to 4 per million per year in the general population. The "watch and wait" policy is the standard of care for newly diagnosed patients in Europe. Patients who progress have first-line medical treatment, surgery is no longer the treatment because there is a high rate of recurrence (> 60%) and radiotherapy is not very suitable because the patients are often young and the tumors are large. Suspicions of drug allergies during a mission expose the doctor to difficulties. This study will improve understanding of the allergy patient's point of view. The end result could be a set of ideas for measures to raise awareness among these patients, and motivate them to undergo an allergological assessment.
