The Phase 1b part of this study is conducted to assess the safety and tolerability of E7386 in combination with pembrolizumab in participants with previously treated selected solid tumors, and to determine the recommended Phase 2 dose (RP2D) of E7386 in combination with pembrolizumab. The Phase 2 part of this study is conducted to assess the objective response rate (ORR) of E7386 in combination with pembrolizumab (melanoma, colorectal cancer [CRC], hepatocellular carcinoma [HCC]) or of E7386 in combination with pembrolizumab plus lenvatinib (HCC) according to Response Evaluation Criteria in Solid Tumors (RECIST) 1.1.
Activation of the RIG-I innate immune pathway and increased expression of tumor antigens and pro-immune genes by DEC-C during Nivolumab treatment may enhance the frequency and activity of anti-tumor immune cells (CD4+ and CD8+ T-cells, NK cells) and reduce the frequency and activity of immunosuppressive cells. This may increase the overall effectiveness and success of Nivolumab treatment. This pilot clinical trial will demonstrate whether combinatorial immunotherapeutic approaches that target epigenetic immune repression and RIG-I activity can favorably alter the tumor immune cell microenvironment and benefit patients with mucosal melanoma.
Skin melanoma is a common form of skin cancer. Its diagnosis is usually clinically suspected and then affirmed by histopathological examination. In some cases, the histopathological analysis of these lesions is equivocal, and the malignant or benign nature of the proliferation is difficult to determine with certainty. In these cases, the use of expensive ancillary tests, which are hardly accessible and take a long time to set up (FISH - CGH-array), is usually required. Anti-PRAME immunostaining, an inexpensive and readily available technic, has recently been described as highly sensitive and specific for diagnosing malignant melanocytic proliferations. Knowledge on its utility for evaluating ambiguous melanocytic neoplasms remains limited. Our study aims to improve the current body of knowledge on the utility of PRAME immunohistochemistry for evaluating challenging samples of melanocytic proliferation. The secondary objective is to determine the PRAME immunoreactivity profile for each histological subtype of melanocytic proliferations (spitzoid tumors, cellular blue nevi, dysplastic nevi ...)
Evaluate PFS of PD-1 Toripalimab Combined With Anlotinib in Subjects With unresectable locally advanced or metastatic acral malignant melanoma
Investigators hypothesize that the introduction of basic science explanations within the instructional design of case-based training in visual diagnostics will improve students' learning curves, retention, and retrieval of knowledge/skill following a washout period. Research question: In a group of medical students with limited dermatological training, what is the effect of integrating biomedical causal explanations of visual criteria during a prolonged case-based skin cancer training program in visual pattern recognition when compared with an identical instructional design without biomedical explanations? How will the displacement of students' cognitive resources from practicing pattern recognition towards understanding the pattern, affect their learning behavior, learning curve (accuracy and time per diagnosis), and retrieval of pattern recognition skills following a washout period? The above-mentioned research questions will be tested through a randomized trial with an allocation ratio of 1:1. All participants will be trained in skin cancer diagnostics through a mobile application that offers simulation training and learning through written modules about the various differential diagnoses. Approximately half of the participants will be subject to a written content that displays the dermoscopic visual criteria without an explanation while the remaining half will be subject to the dermoscopic criteria + an explanation of the underlying cause. The training program consists of 500 training cases, a 14 day wash-out period, and a final training session of 100 cases.
