Using Informatics to Enhance Care of Older Emergency Department Patients

Start date: January 2005
Phase: N/A
Study type: Interventional

The subjects in this study are physicians working in the Wishard Emergency Department. The purpose of this study is to measure the extent to which information technology (i.e.-computers) improves emergency department care. The objective of our study is to evaluate the use of informatics in the emergency department and specifically to determine if computer reminder systems: 1) reduce the number of unsafe medications prescribed to older adults, 2) assist in more safely dosing of medications to adults of all ages, and 3) increase influenza immunization of eligible older patients in the emergency department. Interventions: The interventions in this study are computer reminders. When releasing patients from the emergency department, physicians currently write all release orders, including prescriptions, on a computer order entry system that is linked to the Regenstrief Medical Record System. The computerized order entry system will be programmed so that physicians randomized (randomly placed) into the intervention group, the group that will receive the intervention, they will receive one of three types of reminders: 1. The medication prescribed is generally considered unsafe for use in older patients. The reminder will then list appropriate alternatives for this medication. 2. The dose of the prescribed medication is excessive and should be adjusted for the patient's creatinine clearance (or kidney function). 3. This patient may be eligible for influenza vaccination. The physician will then choose to order or disregard the recommendation. The computer system will automatically record what the physician selected to do. The general outcome of interest is the extent to which the electronic reminders successfully improved physician practice in the emergency department setting. This outcome will be compared to physicians who were randomized to the group that did not receive the reminders (the control group).

NCT ID: NCT00294866 Completed - Kidney Failure Clinical Trials

Effect of Paricalcitol on Markers of Inflammation in Hemodialysis Patients

Start date: March 2006
Phase: Phase 4
Study type: Interventional

Studies have shown that patients with ESRD on hemodialysis have high levels of inflammatory markers which may contribute to the high rates of cardiovascular disease and mortality seen in these patients. Vitamin D use in dialysis patients has been shown to have a survival benefit, with paricalcitol at advantage over calcitriol. Since there is some evidence for involvement of the vitamin D receptor in inflammation, this study is designed to look for an effect of paricalcitol on markers of inflammation in hemodialysis patients.

NCT ID: NCT00291863 Recruiting - Peritoneal Dialysis Clinical Trials

Simvastatin Effect on End Stage Renal Failure Patients Treated by Peritoneal Dialysis

Start date: February 2006
Phase: Phase 4
Study type: Interventional

This study will analyse the effect of simvastatin on endothelium dependent venodilation in chronic renal failure patients treated by peritoneal dialysis. The hypothesis is that patients will have a greater endothelium dependent venodilation after four months of simvastatin use.

NCT ID: NCT00291720 Completed - Clinical trials for Cardiovascular Disease

Is Spironolactone Safe and Effective in the Treatment of Cardiovascular Disease in Mild Chronic Renal Failure?

Start date: April 2005
Phase: Phase 2
Study type: Interventional

Patients with kidney failure have a poor survival rate that is due to a much higher than average rate of heart and vascular disease. The reason that kidney failure causes heart disease is unknown but recent research suggests that a hormone called aldosterone, which is increased in patients with kidney disease may damage the heart and blood vessels. The investigators propose, using a randomized blinded trial, to find out whether drugs that inhibit the actions of aldosterone have beneficial effects on the cardiovascular system in patients with kidney failure

NCT ID: NCT00291603 Completed - Clinical trials for Kidney Failure, Chronic

Short Term Effects of FX Dialysers on QOL and Inflammation

Start date: February 2006
Phase: Phase 4
Study type: Interventional

Background: The new hollow fibre FX-class of dialysers (Fresenius Medical Care, Bad Homburg, Germany) features a number of technological improvements that may benefit the patient. This includes the use of the advanced high-flux polysulfone membrane, Helixone®, which has an extremely high endotoxin retaining capability. Theoretically leading to reduced systemic inflammation in the patient, which is an important factor for morbidity and mortality with dialysis. The dialysis membrane is the first to be manufactured using membrane-spinning procedures (nano-controlled spinning technology) that enables the membrane to be modulated at the nano-scale level. The resultant membrane is able to extremely efficiently remove middle molecules, along with minimal loss of albumin. These features may lead to improved patient outcomes, including reduced systemic inflammation and improved quality of life. Aims: 1. To assess the short-term effects of the FX-class Dialyser on quality of life in stable haemodialysis patients 2. To assess the short-term effects of the FX-class Dialyser on inflammatory markers in stable haemodialysis patients.

NCT ID: NCT00289614 Completed - Kidney Diseases Clinical Trials

Patients With Renal Impairment and Diabetes Undergoing Computed Tomography (CT)

Start date: January 2006
Phase: Phase 4
Study type: Interventional

The main purpose of this study is to compare the incidence of contrast induced nephrotoxicity following the administration of Isovue (iopamidol) or Visipaque in diabetic patients with moderate to severe chronic renal impairment undergoing clinically indicated contrast enhanced multidetector computed tomography (MDCT). Serum creatinine will be measured before and up to 48-72 hours post dose.

NCT ID: NCT00288613 Completed - Clinical trials for Kidney Failure, Chronic

FREEDOM: Following Rehabilitation, Economics and Everyday-Dialysis Outcome Measurements Study

Start date: January 2006
Phase: Phase 4
Study type: Observational

Purpose The purpose of this study is to compare the economical impact and clinical parameters of short daily hemodialysis using the NxStage® System One hemodialysis device with thrice-weekly conventional in-center dialysis using a matched cohort from the US Renal Data System (USRDS) database.

NCT ID: NCT00286273 Completed - Clinical trials for Kidney Failure, Acute

Safety and Efficacy of the Use of Regional Anticoagulation With Citrate in Continuous Venovenous Hemofiltration

Start date: March 2003
Phase: Phase 4
Study type: Interventional

Severely ill patients admitted to the intensive care unit may develop an acute failure of kidney function. To bridge the period to recovery, renal function is temporarily replaced by continuous venovenous hemofiltration (CVVH). To prevent clotting of the hemofiltration circuit, heparin is generally used, providing anticoagulation in the circuit and the patient. As a result, bleeding complications may occur, necessitating the transfusion of blood. Anticoagulation of the circuit can also be obtained with the use of tri-sodium citrate, which provides anticoagulation of the circuit without affecting coagulation in the patient and thus without increasing his/her risk of bleeding. The use of citrate may however cause metabolic complications. Primary aim of the present study is to show in a larger group of intensive care patients whether the use of regional anticoagulation with citrate is safe compared to systemic anticoagulation with the low molecular weight heparin nadroparin.

NCT ID: NCT00285298 Completed - Clinical trials for Kidney Failure, Chronic

Pentoxifylline and Progression of Chronic Kidney Disease in Moderate-to-high Risk Patients

Start date: January 2006
Phase: Phase 3
Study type: Interventional

The purpose of the investigators study is to assess the impact of therapy with Pentoxifylline (PTF), a nonspecific phosphodiesterase inhibitor, on kidney function in patients at high-risk for progression to end-stage renal disease. The investigators hypothesize that therapy with Pentoxifylline will slow progression of kidney disease over time.

Phase: Phase 4
Study type: Interventional

The aim of this study is to test whether withdrawal of calcineurin inhibitors, followed by treatment with sirolimus, may improve renal function in renal transplant recipients with chronic allograft nephropathy.
