Trans Cutaneous Electrical Acupoint Stimulation Versus TENS On Blood Pressure In Primary Hypertension Patients

To compare between the effects of transcutaneous electrical acupoint stimulation and TENS on blood pressure and quality of life in patients with primary hypertension BACKGROUND: The current American College of Cardiology and the American Heart Association (ACC/AHA) 2017 guidelines define hypertension as systolic BP (SBP) of ≥130 mmHg or diastolic BP (DBP) of ≥80 mmHg. In adults, an increase of 20 mmHg in SBP or 10 mmHg in DBP is associated with more than a two-fold increase in mortality owing to stroke and other cardiovascular diseases. Hypertension accounts for 13% of premature deaths worldwide and is the third leading cause of disability-adjusted life years (Shah et al., 2022). Transcutaneous electrical nerve stimulation of the Neiguan (P6) point with acupressure is possibly affecting the blood pressure by protecting the myocardial tissues and influencing the nervous system through regulating the autonomic nervous system function and reduction in sympathetic activity. (Hassanein et al., 2021). HYPOTHESES There will be no a significant difference between the effects of transcutaneous electrical acupoint stimulation and TENS on blood pressure and quality of life in patients with primary hypertension RESEARCH QUESTION: Is there a significant difference between the effects of transcutaneous electrical acupoint stimulation and TENS on blood pressure and quality of life in patients with primary hypertension? Evaluating equipment, pre-post intervension outcomes: 1. BP by Digital Sphygmomanometer 2. quality of life by The SF-12 health status questionnaire 3. nitric oxide level in blood by nitric Oxide analysis. 4. sleep quality. by Pittsburgh Sleep Quality Index (PSQI) 5. exercise capacity and endurance by Thirty-Second Sit-To-Stand Test (30 STST)

Caudal Block Versus Quadratus Lumborum Plane Block in Lumbar Surgery

Postoperative pain is a common complication after lumbar spine surgery due to inherent tissue damage during surgical procedures. Many analgesic options have been explored. Opioid analgesics carry the risk of respiratory depression. Nonsteroidal anti-inflammatory drugs (NSAIDs) are popular and widely used as first-line treatment for acute pain following spinal surgery, but can provide inadequate analgesia. High doses of NSAIDs have also been linked to non-unions in spinal fusion surgery. Patient-controlled analgesia or epidural injection analgesia are usually used. One of US guided regional anesthesia methods is the ultrasound-guided caudal block that was first described by Klocke and colleagues in 2003. One of the more recent techniques that has been described recently is quadratus lumborum plane block(QLB),that is posterior abdominal wall fascial plane block first described by Blanco in 2007.In this study we will compare between analgesic effect of ultrasound guided caudal block to that of ultrasound guided quadratus lumborum plane block as well as safety of both modalities in adult patients undergoing lumbar spine fixation surgeries.

Low levels of physical activity and sedentary lifestyle have become a major public health problem contributing to the prevalence of many chronic diseases and reduced life expectancy worldwide, and also increase the risk of heart disease, stroke, hypertension, type 2 diabetes, dementia, depression, excessive weight gain, and breast and colon cancer. Physical activity improves functional health and quality of life. (Iva Sklempe Kokic et al., 2019). In Middle East, Insufficient physical activity and sedentary behavior are key risk factors for obesity and other diseases leading to premature mortality. Regular physical activity can improve self-esteem, cognitive performance, and academic achievement in young people. (Sonia Chaabane et al., 2020). University students, due to the time spent in classes, studying or in front of computers, form part of the population most at risk of adopting sedentary behavior. Different studies have shown that the transition to university studies is usually associated to a decrease in physical activity and an increase in sedentary behavior as a result of lifestyle changes, as well as psychosocial factors. (Cotton et al., 2016). Exercise is often perceived as boring and hard, thereby causing adults and students shy away from physical activity -related behaviors after long days of work and/or school. Instead, individuals are more interested in leisure activities, such as video games, Thus, the combination of video games and engaging in physical activity (e.g., virtual reality -integrated exercise) may trigger their interest and improve their physical activity behavior. (Jiali Qian et al., 2020). So, this trial aim to find if the virtual reality exercise as a new modality will make sedentary physical student more interest to adherent to exercise than conventional aerobic training so makes them more physically active which will enhance their academic performance and endurance.

One of the most serious complications in the critically ill patient complaining of bulbar palsy is aspiration pneumonia. These patients are scheduled for nasogastric tube feeding trying to avoid recurrent aspiration and subsequent aspiration pneumonia. Even though, the risk of aspiration is still present. Putting a total parenteral nutrition regimen for these patients is a supposed strategy to avoid aspiration pneumonia till recovery from bulbar palsy or planning for tracheostomy and or feeding gastrostomy.
